The primary purpose of the Trained Medication Aide is to administer prescribed medications and limited topical treatments to residents, under the supervision of a registered nurse or monitoring by a licensed practical nurse. The Trained Medication Aide assists the licensed nurse in providing safe and ethical care for clients as well as completing other delegated nursing tasks. Conduct consistently reflects the facility’s mission and values.
- Dispense oral, inhalation, suppository, eye drops, ear drops, or topical, if the medications are regularly scheduled.
- Dispense PRN (as needed) medications if the administration of the medication is authorized by a nurse per facility policies and procedures.
- Observe patients for drug reactions and report any problems to the licensed nurse.
- Performing tasks associated with medication administration, including patient vital signs, calculate medication dosages and stock the medication cart.
- Recording medication administration according to facility policies and procedures.
- Seeks guidance from the nurse as needed to perform tasks safely and efficiently.
- Completes procedure of checking the resident’s medication record per facility policies and procedures.
- Preparation of the prescribed medication for administration per facility policies and procedures.
- Assists residents with self-administration, as directed by a licensed nurse.
- Completes documentation after administration of the date, time, dosage, and method of administration of all medications, or the reason for not administering the medication as ordered in the electronic medical record, Matrix Care.
- Assists residents with personal cares including bathing, dressing, grooming and toileting with focus on resident-directed care.
- Make every effort to meet resident’s needs by carefully following their written plan of care.
- Assist with efforts to improve resident outcomes through ambulation training, bowel and bladder retraining programs, range of motion, hydration and nutrition monitoring, pain control efforts and repositioning schedules; and document appropriately.
- Follows all shift routines, policy and procedures regarding accurate performance of vital signs, weights, etc.
- Delivers meals and assists residents with eating. Records accurate food and fluid intake, as assigned. Report any chewing or swallowing difficulty to the nurse.
- Assist the resident with the restorative nursing program including but no limited to, passive and active range of motion, ambulation with appropriate assistive devices and use of transfer belt.
- Assists the residents in transferring using a transfer belt, device or appropriate lift maintaining resident safety at all times.
- Reports promptly to the nurse any changes in the resident conditions.
- Answers call lights promptly and determines resident needs.
- Ensures that residents are clean, neatly dressed, and well-groomed.
- Provides any information on changes in skin conditions to the nurse.
- Observe and report any noted changes in mental status to the nurse.
- Report to charge nurse all incidents, accidents that occur involving residents, and visitors.
- Follow standard precautions, facility safety policies, and works within OSHA guidelines.
- Attends in-services education and staff meetings as required.
- Treats and ensures that residents are treated with respect and dignity.
- Protects organization’s value by keeping patient information confidential.
- Immediately reports any suspected abuse or neglect per facility policy.
- Remains aware and takes special precautions to ensure a clean safe environment for residents and staff.
- Reports any equipment malfunctions to designated employee, including knowledge of Lockout/Tagout Policy and Procedure.
- Accurate documentation of vital signs, weights, intake and output per facility protocol.
- Attend designated nursing meetings, as scheduled.
- Communicate to nurses any changes in resident conditions, complaints of pain and/or mobility changes.
- Practice consistent infection control using Standard Precautions.
- Practice good body mechanics to avoid injury. Adhere to minimal lift policy and follow transfer instructions as outlined by the resident’s care plan. Communicate with nursing any changes in residents’ transferring abilities.
- Complete required documentation immediately after providing cares
- Participates in QAPI and Safety meetings as directed.
- Other duties as assigned.
- Completion of approved Nursing Assistant training program followed by an approved Trained Medication Aide certification program.
- Successful completion of written and skills exam, as provided from a State of Minnesota approved testing location and be in good standing with Minnesota Nursing Assistant Registry.
- Must be able to pass a State of Minnesota Department of Human Services criminal background study, as well as a United States Office of the Inspector General Exclusion search.
Ability to endure prolonged walking, sitting, standing, use of pulling, bending, and stooping movements. Must be able to lift a minimum of 50 pounds. Frequently pushes or pulls residents 300-600 feet with weight resistance equivalent to 35 pounds. Frequently twists, stoops, bends, squats, kneels. Occasionally lifts and carries equipment and supplies weighting up-to 35 pounds. Frequently reaches above or at shoulder level. Ability to adapt to changes in daily work hours and schedule.
- Must be able to clearly communicate in English orally and in writing with residents and staff.
- Must be able to read small print in typed and written material.
- Possesses necessary skills to continuously comprehend and communicate with residents, co-workers, and to respond to emergency alarms.
- Identifies residents, call lights, unusual occurrences on unit
- Concentrates with frequent interruptions and remembers multiple messages, information and task assignments to perform a variety of tasks during an eight hour or longer periods of time.
