Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in a Nursing Supported Group Home licensed by the Arizona Department of Health Services (AZDHS) and funded by the Division of Developmental Disabilities (DDD). This role is vital in providing high-quality nursing care and support to individuals with developmental disabilities, ensuring their health, safety, and well-being in a home-like environment.
Key Responsibilities:
- Provide direct nursing care in accordance with the residents' care plan, under the supervision of a Registered Nurse (RN) and in compliance with AZDHS regulations and DDD policies.
- Monitor and document the health status and changes in the condition of individuals, reporting significant changes to the supervising RN or healthcare provider.
- Administer medications and treatments as prescribed, ensuring proper documentation and adherence to safety protocols.
- Assist with activities of daily living (ADLs), including personal hygiene, feeding, and mobility, with a focus on promoting independence and dignity.
- Develop and maintain positive relationships with residents, their families, and other healthcare team members to ensure a holistic approach to care.
- Participate in care planning meetings, contributing insights and observations to enhance care plans and interventions.
- Provide education and support to residents and their families on health-related issues and wellness strategies.
- Ensure the living environment is safe, clean, and conducive to the residents' health and comfort.
- Respond promptly to emergencies, providing appropriate interventions and coordinating with emergency services when necessary.
- Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, and participating in professional societies.
Qualifications:
- Current Arizona Licensed Practical Nurse (LPN) license in good standing.
- Experience in developmental disabilities, long-term care, or a related healthcare setting preferred.
- Knowledge of AZDHS regulations and DDD policies related to nursing supported group homes.
- Strong clinical skills with the ability to assess and respond to the needs of individuals with developmental disabilities.
- Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, and team members.
- CPR and First Aid certification.
- Must pass a background check and have a clean driving record.
