Essential duties:*
- Understanding the medical condition and treatment plan of the patient facilitates coordinating appropriate post-discharge care and services.
- The Case Manager candidate should assess the patient's physical, psychological, and social needs to create a discharge plan addressing their well-being comprehensively.
- Effective communication with patients, families, outside agencies, and the healthcare team is crucial for explaining discharge plans, addressing questions, and concerns.
- Connecting patients with community resources like home health care or support groups facilitates a successful transition.
- Understanding and respecting patient cultural backgrounds is vital for providing patient-centered care.
- Keeping accurate records of discharge plans and services provided ensures continuity of care.
- Identifying potential discharge barriers and finding solutions to overcome them is crucial for successful patient transitions.
- Showing empathy towards anxious patients builds trust essential for successful transitions.
- Good time management ensures prompt arrangements in the often-tight timelines of discharge planning.
- Collaborating effectively with other healthcare team members ensures coordinated care for patients.

- Qualifications:
- Proven experience in addiction case management, preferably in substance abuse treatment.
- Strong communication and interpersonal skills to build rapport with clients and multidisciplinary teams.
- Compassion, empathy, and sensitivity when working with clients facing addiction challenges.
- Effective problem-solving abilities and professionalism in handling sensitive situations.
- Familiarity with relevant laws, regulations, and ethical standards related to addiction treatment and client care.
Education:
High School Diploma or Equivalent required.
Bachelor's degree - from an accredited institution with a major in chemical dependency, psychology, social work, counseling, specialty in the human services or another related field preferred.
