SUMMARY OF POSITION RESPONSIBILITIES:
Under the direction of the Program Director of Westside Community Services AIDS Case Management Program, the Registered Nurse will practice in a community-based setting within the San Francisco HIV System of Care. The Registered Nurse will be responsible for direct nursing care, including, but not limited to, assessing, planning, conducting, and coordinating blood specimen collection for home-bound patients. In addition to direct nursing care, the Registered Nurse will be responsible for administrative duties related to the Mobile Draw Project. Duties related to the Mobile Blood Draw Project include but are not limited to:
Operate independently, competently, and efficiently and to manage workload and service delivery demands.
Perform essential nursing functions including: conducting basic physical- and psychosocial assessments; perform HIV medication reconciliation and perform other essential duties as needed.
Conduct a high volume of venipunctures on patients in residential settings. This includes the ability to: competently utilize phlebotomy equipment; collect, label, handle, and respond to common venipuncture reactions; transport and deliver specimens according to established protocols within a set time frame.
Plan and organize daily assignments, including contacting clients directly to coordinate and conduct home based specimen collection.
Collaborate with referring agencies, including clinical programs operated by the San Francisco Department of Public Health, University of California San Francisco, and other Community-Based Organizations. This includes coordination of client referrals, service delivery, and providing follow-up as needed. In addition, this may include coordinating and managing materials, equipment, and other supplies to conduct home based specimen collection Have familiarity with neighborhoods and layout of the City of San Francisco to organize and cluster home visits to minimize travel time and maximize productivity. This may also be achieved with ability to utilize navigating software applications, such as maps and/or GPS.
Have a strong ability to learn and utilize multiple Electronic Medical Records and/or Client Services Databases to record care and services provided. Client Services Databases to record care and services provided.
Proficiently enter, manage, and report on programmatic outcomes, including, but not limited to: keeping an up-to-date registry of referrals, upcoming service appointments, services rendered, productivity, travel time, etc.

Responsible for an active participation in the quality assurance process.
Have possession of a working cell phone with a sufficient data plan
Exhibit Cultural Competency and have the ability to work with diverse populations including, but not limited, to People of Color, LGBT community members, elders, persons with histories of incarcerations, persons who use substances, and people living with various medical conditions such as HIV, mental health diagnoses, or other comorbid conditions. Must exhibit cultural competency, cultural sensitivity and humility while having the ability to work with diverse populations including, but not limited, to People of Color, LGBTQQI community members, elders, persons with histories of incarceration, persons who use substances, and people living with various medical conditions such as HIV, mental health diagnoses, or other comorbid conditions.
Bilingual Spanish-English is preferred but not required.

LICENSURE AND MINIMUM QUALIFICATIONS REQUIREMENTS:
Meets legal requirements to practice as Registered Nurse (RN) in the State of California, and in possession of a valid RN License as determined by the California Board of Registered Nursing.
Graduation from an accredited school of nursing.
Cardiopulmonary Resuscitation (CPR) Certification. PHYSICAL AND VISUAL ACTIVITIES: Mild to medium to heavy work requiring prolonged or repeated standing, walking, climbing, stooping, kneeling, crouching, lifting, carrying to a maximum of up to 40 lbs.
