Progressive Living Solutions is a local Alexandria residential adult group home provider supporting high functioning adults with mental illness and other disabilities. Currently accepting applications for our Assistant Program Coordinator opportunity, to improve upon our current quality service program. Our residents are kind individuals and there are no threats of physical injury while on shift.
General Description of Duties:
Participate in new resident admissions as assigned by the Program Coordinator, including pre-screening, in-person/digital interview, document preparation, admission proceedings, and completing the appropriate program checklist.
Review resident program files to assure accuracy & adherence to 245D/agency standards. As assigned, prepare program content for care team meetings, and assist with internal charting functions.
Review Medication Administration Records (MAR) on a monthly basis or as needed, for accuracy and completeness. Review new MAR each month.
Add or discontinue medications as directed by resident physicians or mental health providers.
Assure that resident medications are administered in compliance with current orders. Work with pharmacy providers to address any medication concerns.
Process the bi-weekly labor Schedule for both PLS markets. Ensure all shifts have sufficient coverage by trained Direct Care staff whom are capable of performing their assigned duties. Address Scheduling emergencies during scheduled hours and after hours. Fill in for vacant shifts when appropriate.
Perform CNA cares for identified residents, in compliance with their program medical service plans. Includes nail trimming, hygiene, showering/bathing, elimination assistance, wound care, ADL support, and other appropriate cares.
Correspond with the resident’s physicians and mental health providers.
Assist with staff training as assigned by the Program Coordinator.
Other duties as assigned by the Executive Director of Program Coordinator.
Employment Requirements:
- Two+ years’ experience as a provider of direct care services to our target population.
- Two+ years of experience as an CNA in the mental health field is preferred.
- High school diploma and minimum of an AA or AS degree or equivalent experience and education is preferred.
- Must be 18 years of age or older.
- Driver’s license in good standing.
