Job Summary:
Whittier First Day is seeking a dedicated and compassionate Case Manager to join our team at our Whittier Shelter. The Case Manager will play a critical role in providing support and guidance to individuals and families experiencing homelessness. This role involves assessing clients' needs, developing personalized service plans, and connecting clients with resources and services to help them achieve stability and self-sufficiency.
Key Responsibilities:
Client Assessment and Planning:
- Conduct initial intake assessments to understand clients' backgrounds, needs, and goals.
- Develop individualized case management plans that address clients' housing, employment, healthcare, and other needs.
Resource Coordination:
- Connect clients with appropriate community resources, including housing, employment services, healthcare, and social services.
- Collaborate with external agencies and partners to ensure clients have access to a wide range of support services.
Client Advocacy and Support:
- Advocate for clients' rights and ensure they receive fair treatment and equal access to resources.
- Provide emotional support and encouragement to help clients overcome barriers and stay motivated.
Case Monitoring and Documentation:
- Monitor clients' progress and adjust case management plans as needed to meet evolving needs.
- Maintain accurate and up-to-date records of client interactions, services provided, and outcomes.
Collaboration and Teamwork:
- Work closely with shelter staff, volunteers, and other case managers to provide coordinated care.
- Participate in team meetings and contribute to a positive and collaborative work environment.
Compliance and Safety:
- Follow shelter policies and procedures to ensure client safety and well-being.
- Ensure compliance with all relevant laws and regulations, including confidentiality and privacy rules.
Qualifications:
- Bachelor's degree in social work, psychology, human services, or a related field.
- Experience in case management, social work, or a related field, preferably in a homeless shelter or similar setting.
- Strong communication and interpersonal skills.
- Ability to work effectively with a diverse client population and in a team setting.
- Knowledge of community resources and social services.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Basic computer skills for case management documentation and communication.
- Flexibility to work some evenings or weekends, as needed.
Job Type: Full-time
Pay: $50,000.00 - $60,000.00 per year
