RN facilitates continuation of care from hospital to home. Identifies discrepancies between Hospitalist discharge orders and discharge instructions and educates patients on discharged medications, s/s management, diet, and follow up appointments to help reduce readmissions. Assists to improve HCAHPS scores and other quality metrics. Supports the Executive Director of inpatient operations.
ESSENTIAL JOB FUNCTIONS
1. Assists with ensuring that Providers comply with quality risk measures, credentialing requirements and other Performance Standards.
2. Assists with developing provider schedules
3. Calls discharged patients to help reduce readmissions and improve HCAHPS
4. Assists Funeral Homes with Death Certificate Signatures
5. Update quality metric spreadsheet
6. Facilitates Homecare, Hospice, and Medical Equipment completion
7. Adheres to professional standards, policies and procedures, federal, state and local requirements to ensure HIPAA, safety standards and practice protocols are followed.
8. Rounding on patients to introduce our services provided during hospitalization
9. Performs other related duties as assigned.
