Assist the Health Specialist and the Family Development Worker in the planning and implementation of health services, as well as establishing mutually respective partnerships with families and providing support services for the AVANCE - Houston Head Start Program participants.
1. Assists with the monitoring and provision of comprehensive support services for head start families, including health (vision, hearing and dental examination, screenings and transportation), mental health, disabilities, and child and family development as needed, that includes home visits, advocacy, transportation and referral services.
2. Assists with and notifies parents of all missing documents, including but not limited to eligibility and enrollment documents, medical examinations and/or screenings, findings, results, treatments, recommendations, referrals and follow-ups.
3. Maintains and updates family development information and medical data and immunization records of children and families in assigned centers.
4. Assists with implementing preventive health and safety measures while promoting the well-being of children.
5. Assists with dental and medical emergencies, accidents and illnesses of children as assigned by the Health Specialist.
6. Acts as a Liaison between parents, providers, Mental Health, Education, Disabilities and community resources to ensure continuity of care.
7. Counsels parents and interacts with children to promote a positive attitude toward health, dental hygiene, mental health, nutrition and safety.
8. Makes required number of contacts with each family and maintains accurate and timely documentation and narrative reports in families’ file.
9. Participates in meetings and briefings as a contributing member of the Support Services Team.
10. Participates in scheduled case staffing including those with family development worker, introduces family cases for review and input from peer group.
11. Participates in some aspects of the program’s recruitment and enrollment efforts.
12. Prepares a comprehensive monthly report of all service activities, as well as prepare and maintain comprehensive data for reviews, audits, licensing, and/or any other inspections relative to health services.
13. Complies with entry of MIS (PROMIS) data to maintain children’s records, as required.
14. Utilizes personal vehicle for work-related activities, as needed.
15. Becomes thoroughly familiar with Head Start Performance Standards as they relate to this position and all aspects of the program.
16. Researches community resources for additional service opportunities for parents.
17. Attends local, regional and national training activities, as assigned.
18. Assist in parent meetings.
19. Must be able to travel for business related activities.
20. Ability to maintain regular and punctual attendance in compliance with AVANCE-Houston policies and/or procedures.
21. Performs other duties as assigned.
REQUIRED EDUCATION:
High School Diploma or GED.
- Must sign education agreement committing to obtain an active/current Certified Nurse Aide Certificate, Certified Medical Assistant Certificate, or Patient Care Technician Certificate within eight months of hire date.
Diploma and/or degree must be earned from a state accredited or federally recognized education program.
PREFERRED EDUCATION:
Associates Degree in related Health or Social Service field and an active/current Certified Nurse Aide Certificate, Certified Medical Assistant Certificate, or Patient Care Technician Certificate.
REQUIRED EXPERIENCE:
One (1) year experience in providing health related services
PREFERRED EXPERIENCE:
Three (3) years’ experience in providing health related and/or social services.
