As an Admissions Hospice Nurse, you will work under the direction and supervision of the Admissions Director. You will assess patients utilizing the Nursing Assessment to identify the physical, psycho, social, and environmental needs, develop a plan of care, and ensure a timely submission of all paperwork. Just like all our team members, we ask our RNs to uphold core values and standards.
Covering Cook, DuPage, Kane, Kendall, Grundy, Lake, McHenry, and Will Counties.
Job Qualifications:
Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Licensure: Current license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Driver’s License.
Experience: Two years of experience as a Registered Nurse in a hospice care setting is a must. Must have experience in Hospice Admissions.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Proof of current CPR. Must read, write, and comprehend English. Strong communication. Excellent critical thinking and organizational skills.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Pay Rate: $85,000/yr-$90,000/yr BOE
Schedule: Monday- Friday either a 9am-5pm schedule or 10am-6pm schedule
