POSITION SUMMARY
The LPN is responsible for coordination of care between patients, medical providers, and administrative clinical staff. The LPN’s primary responsibilities are providing support for the effective delivery of health care services in the clinic, direct patient care and provider support.

PRIMARY TASKS & DUTIES
Promote health, to prevent disease and help patients cope with illness. Serve as an advocate and health educator to patients, families, and the community.
Prepare patients for and assist with examinations, administer injections to all age groups, assist with lab draws, monitor vital signs, maintain records on the Electronic Medical Record.
Assist with procedures such as pap smears, breast exams and complete plan of care and referrals after visit if necessary.
Assist with medication refills, medical record request and medication authorizations.
Facilitate clinic flow and room turnover
Reviews each patient chart before appointments compiling their disease process and diagnosis noted while updating all testing, screening and vaccinations associated with there diagnosis.
Schedules follow up appointments for the patients and ensure understanding of these dates and times.
Review daily abnormal lab values assigned by the Provider and make appropriate calls, to complete orders related to the lab values. Follow up of these orders are recorded in the EMR and confirmed with ordering provider.
Responsible for taking initial and interim history
Checks equipment and supplies, restocking as necessary and checks emergency kits as directed.
11. Accountable for notifying their supervisor for any emergencies or unplanned changes in
their schedule that requires coverage as directed in employee handbook.
Responsible for maintaining required education.
Must be able to commute to other Genesis Health Care, Inc. locations.
Responds to phone calls in a respectful and timely manner.

POSITION REQUIREMENTS
Education
· Possession of diploma in Nursing. Must be able to speak, read, write, and understand English.
· Must have an unrestricted active license in the State of South Carolina.
Professional
· Four years of professional experience in a Health Care Organization, working directly with Medical Providers and Patients preferred.
· Communication, customer service or working with the public, preferably in a medical care facility.
· Knowledge of medical terminology preferred.
Physical/Environmental
· Ability to interact with computer screen for up to six hours at a time (visual acuity required).
· Must have manual dexterity for use of keyboard. Ability to stand for periods of up to two hours. Ability to communicate via phone, mail and in person to resolve patient issues.
· Capacity to function in a sometimes stressful, multi-tasking environment

Pay: $23.00 per hour
Expected hours: No more than 40 per week
