Lakeshore Home Health Care has extensive experience in providing home health care services for senior, adult and pediatric patients who have a variety of medical conditions and special needs
We are currently hiring several Registered Nurses (RNS) and Licensed Practical Nurses (LPNS) to provide that 1 on 1 care to our medically fragile and special needs children and adults. Many of our clients are dependent on tracheostomies, ventilators and g tubes.
We currently have availabilities in Muskegon, Casnovia, Ada, Belding, Comstock Park, Grand Rapids, Kentwood, Wyoming, Hudsonville, Kalamazoo and Battle Creek.
**Duties:**
- Provide direct patient care in accordance with established policies, procedures, and protocols
-Provide care in accordance with the state Nurse Practice Act and standards for home health nursing
- Assess patient health problems and needs, work with case managers to develop nursing care plans, and maintain medical records
- Administer nursing care to ill, injured, convalescent, or disabled patients
- Collaborate with healthcare professionals to plan, implement, or evaluate treatment
- Monitor patient vital signs and symptoms and report changes to the healthcare team
- Educate patients about health maintenance and disease prevention
- Operate medical equipment such as ventilators, pulse ox machines, concentrators, feeding pumps.
- Maintain confidentiality at all times
-Monitor for any illnesses or changes in condition to report to family and physicians, as well as members of the health care team.
-Utilize standard precautions to promote safety and prevent the spread of infections/illnesses.
**Requirements:**
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Michigan
- Excellent communication and interpersonal skills
- CPR certification required
- Must be able to pass a criminal background check
