Description:
The Case Manager/Discharge Planner/Social Worker will:

Be responsible for coordinating and facilitating the discharge planning process for patients transitioning from our healthcare facility to their homes or other care settings.

Work closely with patients, families, healthcare professionals, Utilization Review staff and community resources to ensure a seamless transition and access to necessary support services.
Requirements:
- Associate degree in social work, nursing, or a related field

One (1) year of experience in diverse clinical settings

Prior case management or discharge planning experience.

Previous experience in case management, discharge planning, or social services in a healthcare setting (preferred)

Prior experience with Electronic Medical Record (preferred)

Basic Life Support (BLS) for Healthcare Providers (preferred)

Working knowledge of federal, state, and local laws that govern health care and case management. Knowledge of community resources.

Knowledge of medical terminology, healthcare systems, and community resources is essential.

Strong assessment and problem-solving skills to identify patient needs and develop appropriate discharge plans.

Excellent communication and interpersonal skills to collaborate effectively with patients, families, and healthcare professionals.

Ability to navigate complex healthcare systems and work under time constraints.

Compassionate and empathetic approach to provide support to patients and families during challenging transitions.

Proficient in computer skills, including electronic health records and Microsoft Office applications.
