Crossroads was founded on May 5, 2005 to serve a patient population battling with opioid addiction. Since its founding in 2005, we have been a physician led healthcare organization. The company has grown rapidly and now includes 90+ treatment centers in 9 states. Each member of the Crossroads team specializes in an area that supports the recovery of over 26,000 patients, monthly. Crossroads' clinical staff includes physicians, pharmacists, counselors, nurses, and other service coordinators. Along with medication management, our staff works closely with each patient to create an individualized treatment plan aimed at building recovery and relapse prevention skills.
Day in the Life of a Patient Navigator/LPN
- Greeting patients and answering clinic phones.
- Assist patients in understanding their treatment options, and the resources.
- Serve as an essential link between patients and all other care.
- Facilitate appointments for consults and support services within established service standards, including social workers, physician, etc.
- Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
- Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management.
- Work with staff and others to collect data, track outcomes, and support strategic planning processes.
- Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, social services.
- Assist Center in stocking supplies and Patient Ambassador team.
- Other duties as assigned.
Education and Experience requirements: Must hold one of the following credentials/experiences:
- RN, LPN, or BSW
- Bachelor’s Degree WITH at least 2 years of lived addiction experience (personal or professional)
- Associate's degree or CMA WITH 4 years of lived addiction experience (personal or professional)
Hours, Schedule, and Travel (if applicable)
Our Patient Ambassadors enjoys an early morning schedule with early afternoon out times. It is expected that Patient Ambassadors have flexibility to support on weekends and if applicable, evenings. Expected hours for this role are 37-40 hours per week. See specific schedule requirements below.
Specific Schedule:
- Tuesday; Galloway 9:30am-1:00pm, travel to Lanoka Harbor 2:30pm-5pm
- Thursday; Lanoka Harbor 10:30am-7:15pm
- Friday; Rio Grande 8:30am-12:00pm, travel to Galloway 1:30pm-5:45pm
