PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.
New York State (NYS) recently announced the availability of $500M statewide from 8/1/24 – 3/31/27 to support Social Care Network (SCN) lead entities that will be responsible for the coordination of social care delivery in various regions across the state. PHS and our WYNYC network recently applied to become a regional in New York City. We seek an experienced project manager to manage our organization-wide SCN implementation planning and readiness anticipating an 8/1/24 go-live.
Position Summary:
Specifically, the Senior Project Director will work in close collaboration with the Chief Strategy Officer / Executive Vice President of Neighborhood Health, Senior Director of Healthcare Innovation and others across the organization to:

Develop and manage the implementation plan to establish our regional Social Care Network (SCN) from inception through execution, utilizing effective project management methodologies while ensuring compliance with PHS policies, New York State contractual requirements, and established timelines.

Coordinate and lead discussions with diverse, cross-functional teams supporting the Social Care Network (SCN), including, but not limited to, Legal, Finance, Contract Administration, Procurement, IT, HR, and the WYNYC network management teams to make key planning decisions and remove implementation barriers.

Participate in and facilitate discussions with external stakeholders, as needed, to provide oversight of their dependent scope, deliverables, and timelines.

Monitor progress against implementation activities, milestones and deliverables with reliable tracking over time to ensure timely completion.

Collaborate with department leads and other subject matters advisors to coordinate and align SCN requirements with business strategies, processes and deliverables.

Track, manage and resolve implementation risks and issues, ensuring timely escalation and resolution.

Serve as the main PHS point of contact to communicate implementation status, risks and issues.

Prepare and distribute high-quality status reports communicating progress against key implementation milestones, issues and risks.

Develop customized views of status reporting and presentations to summarize implementation progress for various audiences, such as the PHS board, senior management team, and WYNYC CBO partners.

Schedule and lead implementation status meetings and discussions.

Support leadership through effective planning, communication and relationship-building to resolve issues and accelerate implementation progress.

Perform other duties and functions as assigned.
Qualifications and Experience:

Bachelor’s degree required.
-
Minimum of 5 years of experience in a significant project management role, including extensive experience in developing and managing integrated project plans.

Demonstrated ability in successfully managing complex, large-scale, multi-disciplinary projects in a fast-paced, dynamic environment.

Experience facilitating cross-functional group discussions for consensus-based decision-making.

High degree of independence, initiative, problem-solving, and time-management skills.

Ability to flexibly adapt to new information and requests, potentially requiring a shift in priorities.

Exceptional listening, writing, oral, and presentation skills.

Excellent team player with substantial experience working in collaborative environments.

High level of self-awareness and diplomacy; comfortable interacting with all levels of management.

Strong ability to quickly learn tasks and new topics and apply critical thinking skills.

Proficient in Microsoft Office 365 Suite applications.

Based in New York City with the ability to travel across the five boroughs as needed.

Dedicated to improving the lives of vulnerable and marginalized communities.
Desired Skills:
-
Project Management Professional (PMP) certification.
-
MPH or related Master’s degree.
-
Foundational knowledge of the healthcare and social service sectors to include a solid understanding of the network of health and human service providers in NYC / NYS and related government agencies.

Prior understanding of Social Determinants of Health is a plus.

This position is contingent upon grant funding. We are proactively recruiting candidates prior to the grant award to expedite the hiring process. While grant funding is not guaranteed, we expect funding to be available until early 2027.
