Foster Care Intake Coordinator

What will you do?
The Foster Care Intake Coordinator is responsible for coordinating services and resources for youth and families being served. This position also provides ongoing support and training to foster parents as well as ensuring that foster parents maintain compliance with established standards.

Update Bed board daily in Client Care Web.

Update Placement discharge log weekly.

Update Foster Parent bed availability weekly and send to staff via email.

Responsible for securing agency respite for current children.

Respond to Client Care Web referrals via email and phone calls requests weekly.

Create Intake packets and obtain intake documents prior to placement. Obtain any outstanding documents at time of mobile intake.

If a child is on medication, send a list of medications and non-script form to the clinic for medical review and signature.

Collect medical/physical documentation from medical provider. If the form is over a year old, please be sure to have the foster parent schedule within 7 days of placement.

Collect dental documentation from the dentist. If the form is over 1 year old, please be sure to have foster parents schedule within 4 weeks of placement.

Schedule or obtain Comprehensive Clinical Assessment.

Create Person Centered Plan and Comprehensive Clinical Assessment and obtain Dr. signature.

Submit auth to Utilization Team, inform team that auth was submitted.

Create child profile in EHR and add goals as well as child to the parent.

Create NCTOPPS for the child. If child has a current NCTOPPS, reach out to MCO to obtain discharge from previous agency so we can complete for our agency.

Create and enter child as well as clinical documents into Client Care Web prior to placement.

Once the child is placed send a face sheet along with daily stipend to Foster parent, licensing and Care Coordinator.

Schedule orientation with new foster families.

Seek out placement for current children that disrupted placements.

Attend meetings for transfer families to gather all intake and clinical documentation prior to date of transfer.
Requirements:
What qualifications do you need?

Completion of at least a high school diploma or equivalent

Minimum of 1 year experience working in a medical office setting

Experience with Medical Terminology

Must be at least 18 years of age, be able to read, write, understand, and follow directions, has no substantiated findings of abuse or neglect listed on NC Health Care Registry, and no criminal convictions.
