Job Summary
-
The Intake Coordinator manages and oversees all department functions in reception and intake of new Medicare and Non-Medicare referrals, as well as all other activities related to patient care such as lab results, placing Clinical orders, hospital holds, etc.
Responsibilities
-
Manages and oversees patient referrals.

Process Initial Home Health Order to ensure patient receives care within 48 hours.

Coordinates and oversees department functions, duties, and activities for Intake of new referrals.

Acts as a liaison between LORIAN and referral sources to ensure smooth processing of referrals
-
Ensures that all relevant patient information is obtained in an accurate and timely manner, and is entered into HomeCare HomeBase system, in order to meet MEDICARE requirements or other insurances, when patients are referred to LORIAN by referral sources.

Regularly collaborate with the Sales and Clinical management team in reviewing new referrals and determining the appropriateness of any given referral for the agency.

Provides all relevant patient information (i.e. patient personal demographic, history and physical, current medications, physician’s orders, Face to Face (F2F), surgical reports, etc.) obtained upon referral to the Medical Records Department for scanning into patient charts.

Identify, assign and schedule the required Start of Care clinical staff to each patient admitted by LORIAN.

Process all DME requests from clinical field staff to include contacting vendors, faxing orders, and following up with the ordering clinicians once complete.

Serve as a liaison between the DME vendors and field staff.

Receives all processed lab results from community/hospital laboratories (via mail/fax), faxes to ordering physicians and related pharmacies, places copies in case manager LORIAN mailboxes, provides original to Clinical Manager for review and processing

Troubleshoot problems with lab specimens, requisitions, and any other issues that arise in regards to labs from both our internal and external customers.

Calls Discharge Patients in order to perform a Customer Service Survey and to inquire if additional Home Health services are needed.

Assists in patient concerns and immediate needs of the patient and delegates to clinical staff as necessary.

May perform other duties as assigned by management staff.

Knowledge of Lorian policies and procedures.
Required Education and Experience
-
High School education
-
One (1) to Two (2) years of experience in Home Health.

Must have professional and customer-service-driven phone skills
-
Communicates effectively with all customers internal and external in a constructive, goal oriented, and professional manner.

Ability to be flexible, to follow verbal and written instructions, and to work in a team environment.
-
Strong knowledge of Microsoft Office Systems (i.e. Word, Excel, PowerPoint, etc.) and computerized systems in general required.

Proficient in completing routine paperwork, multi-tasking, and providing appropriate follow-up as needed.

Must know health care terminology.

Strong organizational, interpersonal, and communication skills (both written and verbal)

Maintains confidential patient information in accordance with privacy and security standards of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
