Overview:
MANA Family Medicine Har-Ber is looking for a full-time LPN/RN Care Coordinator. Hours are Monday-Thursday 8-5 and Friday 8-12.
This position requires an individual that is self directed with exceptional time management and organization skills, a high degree of professionalism is expected in this role. The ideal candidate will work with the current staff to complete the care team in hopes to better improve patient care and the patient's overall experience.

Responsibilities:
- Promotes patient self-management and empowers patient/ families to achieve maximum levels of wellness and independence

Responsible for working with patient and patient's care team to coordinate change readiness, needs assessment and develop and individualized treatment care plan

Collaborates with the patients, physician, and other care teams’ members in assessing the patient's progress toward individual health care goals

Assesses barriers when patient has not met treatment/ goals, is not following treatment plan of care, or has not kept important appointments
Qualifications:
- Graduation certificate from accredited LPN / RN School of Nursing

12 months related experience and/or training preferred

LPN or RN active license

Prior Family Medicine experience preferred, but not required
