Job Summary:
Social worker develops and provides psych-social services that meet the needs of the ADHC program participants; works effectively to coordinate these services with other services (RN, PT/OT, RD) and activities at the facilities.
Essential Functions:
- Participates in initial and continued assessment of the participant’s needs for the development of individualized plan of care by a multidisciplinary team.
- Observes participants’ mental health status for change when indicated and assists participants in obtaining necessary care.
- Accurately records participant’s progress on flow sheets and progress notes in a timely manner.
- Documents changes in clinician’s orders, submits written additional orders in a timely manner, and documents communication and coordination with other team members.
- Arranges for and assists with screening and other resources requiring outside providers, e.g. DPSS, Alzheimer's Association, Meals on Wheels, ACCESS for transportation services, Asian-Pacific Counseling Services.
- Develop individualized treatment plans with multidisciplinary team member. Coordinates the psych-social program with other Center services and activities in a cooperative, effective way.
- Teaches, supervises, and counsels participants and family members regarding psych-social needs and support of the participant at home.
- Maintains communication with participant’s psychologist / LCSW and keeps him/her informed of changes in mental health status, in order to maximize participant’s IADL.
Skills:
- Proficient in MS Word and Excel
- Knowledgeable of clinical disease entities preferred
- Bilingual in Korean preferred
Minimum Position Requirements:
Education: Bachelor's Degree in social science from an accredited university
Experience: 1 year experience in related field preferred
Licenses/Certifications:
- Valid California Driver's License with good driving record
- Proof of COVID vaccination and TB Test

Pay: $20.00 - $27 per hour
