SALARY INFORMATION :
- $31.05 - $38.42 Dependent on Experience

QUALIFICATIONS:
- Requires current Colorado licensure as a Licensed Clinical Social Worker (LCSW) Licensed Professional Counselor (LPC), or Licensed Psychologist (PSY)
- Master’s Degree or Doctorate Degree in Psychology or related field preferred
- Clinical Social Worker (LCSW) preferred.
- Minimum of 2 to 4 years’ relevant experience in an integrated clinical setting preferred.

BASIC FUNCTION: As a valued member of our Primary Care treatment team, you'll be the hero of mental wellness, identifying, triaging, and providing top-notch mental health treatment to patients of all ages with behavioral health challenges at our clinic locations in Rifle, CO or Parachute, CO. The ideal candidate will cultivate a therapeutic environment within our clinic, where patients feel safe, supported, and inspired to take charge of their mental health.
RESPONSIBILITIES:
- Interviews clients and their families and coordinates and plans programs and activities to identify patients' behavioral health needs and empower them to embark on a journey of healing and growth.
- Provides psychotherapy or counseling to individuals, groups, couples, or families. This includes individual, family and group therapies.
- In addition, provides skill training through psycho-education and patient education strategies and develops specific behavioral change plans for patients.
- Works with Clinic West team to treat and manage patients with emotional and/or health problems efficiently and effectively.
- Assists in the detection of at-risk patients and development of plans to prevent further psychological or physical deterioration.
- Evaluates patient care plans with Clinic West team.
- Teaches patients, families and staff about care, prevention and treatment enhancement techniques.
