JOB SUMMARY:
Works in collaboration with physicians and other medical and social health members to provide general health maintenance and prevention of disease to family planning patients. Care provided is based on protocols approved by the medical director. Care includes complete physical exams, determination of disorders, treatment, or referral. Work in collaboration with the Executive Director to ensure organizational effectiveness, efficiency, safety, and compliance standards are met.

EDUCATION AND EXPERIENCE:
- Master’s degree, post-master certificate or doctorate in nursing required.
- Possess a current unrestricted license to practice as a Nurse Practitioner in the State of Illinois or hold certification by the Physician’s Assistant Association.
- Five years of nursing experience required.
- Knowledge of computerized Electronic Health Record (EHR)
- Excellent public relations skills with the ability to interact with physicians, patients, and staff with a positive outlook.
- Knowledge of:
- needs of patients regarding sexuality
- family planning concepts
- family and interpersonal relationships
- clinic function and ability to work in conjunction with other clinic personnel
- community health resources for alternate health services
- Acceptance of family planning concepts, philosophy, and practices

ESSENTIAL JOB FUNCTIONS:
- Delivers direct care to patients of all ages developing therapeutic plans of care for prevention, management, and maintenance of optimum health for individuals.
- Performs a physical exam of patients, orders and/or conducts and evaluates appropriate tests to determine medical condition/status and to diagnose illness.
- Reviews and interprets pap smears, blood tests and cultures and contacts patients to discuss and arrange treatment if results indicate abnormalities as outlined in the laboratory protocols.
- Manages common gynecological protocols.
- Manages hormonal contraceptive users.
- Manages and evaluates patients for pre-exposure prophylaxis (PrEP) for HIV.
- Performs minor surgeries and procedures: Nexplanon insertion/removal, IUD insertion/removal and acid treatment.
- Provides health education and counseling to those patients seeking family planning services promoting positive health behaviors and self-care skills.
- Makes referrals to health care professionals, specialists, other clinics and community agencies.
- Adheres to the minimum medical guidelines in the Title X Clinical Manual.
- Adheres to State of Illinois statute regarding mandatory reporting of child abuse, child sexual assault and domestic violence.
- Adheres to State of Illinois STI guidelines for treatment and testing.
- Consults with medical directors regarding any abnormal findings not covered by protocol.
- Provides health counseling/education, prevention of STIs and guidance to patients and/or families.
- Prescribed treatments and medications according to medical protocol.
- Identifies and makes medical directors aware of their own abilities and limitations.
- Keeps in confidence any information learned except when mandated by law concerning patients in clinic.
- Provides counseling and teaching about family planning methods to include, but not limited to:
- Hormonal contraceptive methods
- Intrauterine devices
- Diaphragm, FemCap
- Foam, jelly, condom, and other over-the-counter methods
- Natural family planning
- Sterilization – male and female
- Abstinence
- Emergency contraception
- Infertility
- Human sexuality
- Nutrition
- Breast and cervical screening and follow-up
- Sexually transmitted infections, including HIV/AIDS
- Other minor gynecologic concerns
19. Participates as a preceptor and role model for Nurse Practitioner Students.
20. Holds current certification for basic cardio-pulmonary resuscitation.
21. Performs other duties as assigned.
