Licensed Practical Nurse (LPN)

Responsibilities Include:
- Performs selected nursing procedures such as administration of medications and treatments prescribed by the physician.
- Assists with the preparation, implementation and continuing evaluation of the nursing plan of care.
- Observes patient status, changes, etc. and reports pertinent information to the RN case manager.
- Completes documentation in the clinical report and provides report to RN/PSM.
- Participates in case conferences and team meetings.
- Maintains and increases own technical expertise and knowledge through attendance at in-services and other learning opportunities.
- Performs related duties as assigned.
- Rotates weekend and holidays as needed
Required Skills:
- Strong computer skills and the ability to use the Agency's EMR program.
- Strong interpersonal, organizational, time management, and written and verbal communication skills
- Able to work independently and complete paperwork accurately and in a timely fashion
- Must comply with privacy & HIPAA laws. Must be respectful of the patients' rights & treat a client & their belongings with dignity & respect
- Ability to lift a minimum of 50 lbs. and move patients
- Extensive knowledge of basic health care procedures
- Knowledge in measuring and administering medication
- Ability to walk or stand for long periods of time
Requirements:
- Community health experience preferred
- Valid driver's license and automobile with current insurance coverage and in good working conditions available to use for work
- Must be willing to travel throughout the assigned county

- Current Practical Nurse license in the State of New York
- Graduate of an accredited school of Practical Nursing
- High School diploma or GED

Salary: $25.00 - $27.00 per hour
