40 Hour- Full Time Position- Monday thru Friday
(Hours/Schedule Negotiable-split shift 1st into 2nd preferred)
Point Click Care Experience Preferred. Valid RN license in the State of CT required. CPR certification required. Skilled Nursing Facility experience preferred. Clinical Management Experience, minimum two years. Responsible for over all management of sub acute rehab 30 bed wing. Salary Negotiable. No weekends. On-Call required.
NO COST HEALTH INSURANCE
· For employee only coverage, the employer contributes a total of $3,700 to the HRA, which can be used to cover the $2,800 deductible, with $900 left over for copays and coinsurance.
· For employee plus dependent coverage, the employer contributes a total of $5,250 to the HRA, which can be used to fully cover the employee's deductible and most of the "family" deductible (i.e. $5,250 of the $5,600 deductible).
What retirement benefits are offered?
RNs can earn a whopping 13.5% of their gross earning each paycheck towards their future retirement – all fully vested when contributed. Colonial contributes 9.5%of each employee’s gross earnings starting with the first paycheck. Colonial also matches employee contributions. Colonial matches 100% of the first 3% contributed by an employee and 50% of the next 2% contributed by an employee, for a total matching contribution of 4%.

DUTIES:
Manage CNAs & LPNS on 30 bed unit
Supervise care of 30 Residents
Attend Medicare meeting
Implements, coordinates and supervises compliance with clinical policies and procedures using an interdisciplinary team process.
Reviews unit’s change of condition report daily and supervises/coordinates nursing services and care plan interventions.
Addresses clinical issues, goes on rounds and obtains MD orders. Follows up, notates and post MD orders.
Participates in development of a written plan of care (preliminary and comprehensive) for each resident from the MD orders that identifies the problem/needs of the resident indicates the care to be given, goals to be accomplished and which professional service is responsible for each element of care. Reviews resident care plans for appropriate resident goals, problems, approach, and revision based on nursing needs.
Serves as the point person for resident’s family. Addresses any concerns, complaints, grievances the resident or their family may have.
Ensures CNA (POC) ADL records and CNA assignments are accurate and complete. Monitors for complete documentation. Follows up with staff if incomplete.
Ensures that all RN’s and LPN’s on each shift comply with written procedures for the administration, storage, and control of medications and supplies. Ensures all RN’s and LPN’s on each shift are accountable for complete and accurate summaries for each resident. Follows up as necessary to ensure accuracy and completeness.
Monitors medication pass, treatment schedule and corresponding documentation to ensure compliance with applicable regulations and policies.
Provides nursing support on floor and direct care as necessary.
May serve as on-call Nurse Supervisor.
Performs administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary
Participates in arranging for and making resident appointments for diagnostic and therapeutic services.
Ensures that discharge paperwork is complete and resident is cleared for a safe discharge including but not limited to, (PCP appointment, Medications or Prescriptions provided upon discharge, teaching of self administration of medications to treatments, teaching to family/friend/or caregiver is provided and documented).
Review all labs, x-rays, consults, to ensure abnormalities are reported timely to the attending physician.
Survey Preparation: Ensure nursing standards and practices are met in order to ensure facility is below State average deficiency number with no sub standard deficiencies and no immediate jeopardy
