I. Summary
The Coordinated Entry System Behavioral Health (CES BHC) Coordinator at the Human Services Campus serves as a liaison between homeless Mercy Care members accessing the CES and behavioral health services systems. The CES BHC will employ a collaborative approach to the implementation of these services to ensure client choice and partner voice, implement program policies in coordination with funder and partners, provide assistance, build and maintain relationships with partner agencies, monitor quality assurance and agreements with participating partner agencies, oversee contract compliance and reporting, and ensure advocacy for best placement for members entering the homeless system.
II. ESSENTIAL REQUIRED FUNCTIONS
- Support a caseload of 14-16 clients experiencing homelessness and metal health /substance abuse issues directly referred from the Housing Match Coordinators.
- Assist both GMHSU and SMI eligibility determination processes, provide screening to match members with the best-fit program, and coordinate to maximize usage of available system capacity among all participating service providers.
- Guide the client in obtaining necessary documents for housing, such as: Seriously Mentally Ill (SMI) evaluations, State Identification, Birth Certificate, documentation of disability, etc.
- Conduct vulnerability assessments (VI-SPDAT and SPDAT) as needed.
- Provide a strong understanding of BH processes and procedures, and serve as a resource for CES partners.
- Facilitate warm hand-off into housing programs when vacancies arise and a referral is provided from the Housing Match Coordinators.
- Assist HSC staff and partners to verify enrollment with Mercy Care.
- Assist in the data entry on BH enrolled into agency and regional databases, including HMIS.
- Assist in establishing and maintaining relationships with both homeless services and behavioral health organizations for outreach coordination, partnership agreements and compliance.
- Collect client demographic information and obtain appropriate releases of information.
- Assist Navigation Manager in ongoing program review to ensure that program standards are maintained.
- Provide services in a manner congruent with harm reduction theory, progressive engagement, low-barrier eligibility, trauma informed care and national best practices.
- Coordinate with CES partner agencies and behavioral health system of care to ensure regional consistency of access site and service delivery.
- Works closely with HSC Staff providing in-reach, engagement, housing and housing match services.
- Maintains adherence the campus policies, attends yearly updates and any specific trainings established for their department. Promptly reports any violations to their immediate supervisor.

III. Minimum Qualifications
Bachelor’s Degree in Human Services is required. Minimum of 2 years’ experience working with mental health service delivery in the state of Arizona. Experience working with the homeless population a plus. Knowledge of healthcare and the behavioral health service system and resources is essential. Must be able to effectively communicate with clients from diverse backgrounds including those who are experiencing mental health, substance use, traumatic, and medical barriers; and must model effective engagement strategies.
Must be able to professionally communicate and maintain product relationships with colleagues and partner agencies; work in a fast paced environment; work as part of a team; use sound judgment; demonstrate personal responsibility and integrity show initiative; and an ability to work interconnected. Sensitivity to cultural diversity, lifestyle issues is essential.
IV. Required Functional Abilities.
- Effective written and verbal communication skills (Spanish a plus).
- Visual acuity sufficient to maintain systems of records and reports containing computers-generated and handwritten documents.
- Valid Arizona State Driver’s license as this position will have a significant amount of travel involved.
- Ability to handle sensitive material, maintaining the highest level of confidentiality and display sound judgement.
- Mobility sufficient to conduct regular duties within a normal office environment.
- Ability to lift up to 20 lbs. in order to safely manage office supplies and equipment.
