Youth Eastside Services (YES) is a leading provider of youth and family behavioral health services in East King County.
YES provides evidence-based mental health counseling, substance abuse treatment, and psychiatric services to help children, teens, and families struggling with depression, grief, trauma, substance abuse, cultural- and gender-identity issues, anxiety, ADHD, autism, eating disorders, low self-esteem, and much more.

Job Summary
The Care Coordinator is responsible for providing interim services to new clients as well as ongoing case management services to individuals participating in behavioral health treatment including screening, referrals, and coordination of care. This position works closely with an inter-disciplinary Care Team.
This position is highly visible and always requires a professional demeanor and clear communication. The nature of this position is fast paced, requires the ability to collaborate with others, a high level of attention to detail, the ability to manage multiple projects, frequently changing priorities, short deadlines, and the use of multiple computer programs.
Primary Job Functions
- Provide case management services including screening, referrals, and facilitating access to shelter, food, clothing, medical care, psychiatric treatment, financial assistance, education/vocation, social activities, and transportation
- Provide life skills education
- Provide interim services to new clients
- Manage incoming referrals from a variety of external sources
- Complete written and verbal communication with outside providers (i.e. court, primary care)
- Develop and maintain collaborative relationships with community partners
- Act as liaison with community resources
- Advocate for clients (landlord, school, etc.)
- Communicates with hospital personnel to facilitate discharge planning
- Participates in interdisciplinary Care Team meetings
- Deliver internal training about available community resources
- Complete accurate and timely documentation
- Conduct monthly file reviews
- Adhere to all Federal, State, and Local confidentiality requirements and guidelines
- Work in conjunction with the Substance Use/ Co-occurring Disorders team.
- Additional duties as assigned which may include but are not limited to:
- Parent Support
- Family therapy coordination
- Group coordination and/or co-facilitation
Requirements
- Bachelor’s Degree in Behavioral Health or
- Substance Use Disorder Professional (SUDP) of (SUDPT)
- Associates Degree in Behavioral Health plus 2 years’ experience
Knowledge & Skills
- Ability to work effectively in cross-cultural situations
- Strong ability to work with a diverse Eastside community including race, ethnicity, gender, LGBTQ+, religious and socioeconomic status
- Experience working with children, youth, and families
- Strong verbal and written communication
- Customer service mindset
- Aptitude to work in a fast-paced environment, prioritize workload, complete assigned tasks, and maintain poise under pressure
- Dependable, receptive to change, willing to learn, cooperative approach to problem-solving, flexible team player
- Effectively work in both a team environment and independently with minimal oversight
- Computer literate and able to use Electronic Health Records and other software programs with basic training
- Experience working in a Substance Use/ Co-occurring Disorders interdisciplinary team.

- Ability to work evening hours and adjust work schedule as necessary to meet business needs
