Responsibilities:
- Conduct comprehensive assessments of patients' medical, social, and psychological needs
- Develop individualized care plans based on assessment findings
- Collaborate with healthcare professionals to coordinate and facilitate patient care
- Monitor and evaluate patient progress towards achieving goals outlined in the care plan
- Advocate for patients and ensure their needs are met within the healthcare system
- Provide education and support to patients and their families regarding treatment options and resources
- Maintain accurate and up-to-date documentation of patient interactions and interventions
Qualifications:
- Bachelor's degree in nursing or related field (required)
- Current licensure as a registered nurse (RN) (required)
- Experience in acute care or critical care setting (preferred)
- Knowledge of medical terminology and physiology
- Strong assessment and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Familiarity with medical records systems and electronic health records
