Job Summary:
CCCHC is seeking an Administrative Care Coordinator who will be responsible for providing administrative support to the Front Office, Medical Assistants and the Clinical Team. The Administrative Care Coordinator position is under the supervision of the Clinic Manager/Designee and requires in depth knowledge of departmental programs (CHDP, VFC, and other programs offered at this site), operations and services, policies and procedures.
Responsibilities:
1. Outreach/Heidis Measures Calls
- Answers incoming calls, takes messages, transfers calls, and provides information to other departments upon request
-Assist with the increase of performance improvement measures.
-Contact patients newly assigned to the clinic via managed care plans to link them to care.
-Enhance communication with patients in regarding services rendered at Complete Care Community Health Center.
2. Medical Records
-Obtains and prepares medical records for the scanning process.
-Determine correct scanning settings to ensure the quality of each document scanned.
-Scan documents into the patients electronic medical record and files them in the appropriate folders.
-Assign all incoming reports, i.e, labs and consults, to the appropriate personnel promptly.
-Assists with processing all release of information requests as per the established workflow.
3. Lobby Ambassador/COVID Screener
-Greeting staff, patients and visitors.
-Taking temperatures with a thermal thermometer.
-Complete COVID-19 screening questions with all individuals entering the building.
-Ensuring all individuals entering the clinics wear masks or hand out masks as necessary.
-Ensuring all individuals use hand sanitizer before entering clinics.
-Help to improve quality service and reduce no-show rates. (encourage patients to confirm appointments via Phreesia.
-Always ensure patient confidentiality and high quali customer service
-Perform all other duties and tasks assigned by the supervisor, physician or manager.
Qualifications & Skills:
-Excellent Interpersonal and organizational skills.
-Excellent telephone skills.
-Ability to multitask while assisting numerous patients,
-Experience with billing and collections in medical settings.
-Great verbal and written communication skills.
-Strong organizational skills with ability to prioritize tasks.
-NextGen (EHR) knowledge preferred.
-Proficiency with computer skills, including proficiency with Microsoft Office applications.
