Duties
- Case Management: coordinates and assesses the long-term health care of patients
- Quality Assurance/IDG: collaborate with physicians and treatment teams to develop patient-care documentation and serve on quality-improvement teams
- Full-time: 85% Office-based; 15% Field Work
Experience
- Minimum of 1 year of experience as a Registered Nurse in Hospice service
- Understands hospice philosophy
- Familiar with HospiceMD and QA processes
Qualifications
- Valid Registered Nurse (RN) license in the state of California
- BLS certification required; additional certifications such as ACLS or PALS preferred
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize patient care
- Strong critical thinking and problem-solving abilities
- Team-player
- Compassionate patient care
