About the Position
A Supportive Hands Hospice RN Case Manager provides care and support through assessing patients, creating care plans, and monitoring patients as they cope with a terminal condition during the end of life.
We seek an experienced Registered Nurse who is team oriented and knowledgeable in Hospice. Compassion, patience, and providing emotional support are vital in this role to both the patient and their family.
Service Area: Phoenix Metro Area
Responsibilities:
- Collaborate with care team (IDT) and other professionals such as the Social Worker, Chaplain, Volunteers to create Plan of Care
- Scheduled in-home visits to evaluate patient's condition and coordinated Plan of Care
- Assess, monitor, and keep a record of patient's condition.
- Notify doctor of change in patients condition when deemed necessary.
- Provide education to patient, family, and care team.
- Maintain precise records and documentation in a timely manner.
Qualifications and Skills:
- Must possess a current RN license.
- 1 - 3 years Hospice nursing experience preferred but not necessary
- Multi-task and time management skills
- Excellent communication skills, both verbal and written
- Naturally compassionate and end of life understanding.
- Exceptional interpersonal skills
- Accurate and timely charting
- Team-oriented
- Ability to handle intense situations.
- Current CPR certificate
- Ability to undergo and pass a background check.
- Valid driver's license and proof of auto insurance
- Reliable transportation
- Ability to navigate a computer/computer program and learn new software as necessary.

Pay: $80,000.00 - $85,000.00 per year
