Job Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a strong background in case management, medical terminology, and patient services.
Case Manager Responsibilities: The Marcann Case Manager is responsible for providing support to individuals and families in need. The Case Manager will evaluate patient needs, develop and implement treatment plans, and coordinate services to meet individual and family needs. This position requires strong organizational and communication skills, as well as the ability to work with diverse clients in a variety of settings. The Case Manager will provide direct services such as assessment and referral, advocacy, and follow-up. The Case Manager will also collaborate with other service providers to ensure comprehensive care for clients.
Case Manager Duties:
- Provide direct casework services to patients which may include assessment, counseling, advocacy, referral and follow up services.
- Develop and implement care plans with patients to identify and address their needs and goals
- Maintain accurate and up-to-date patient records and documentation, billing and ensure compliance with Marcann policies.
- Collaborate with other professionals, including other case managers, to ensure proper coordination of services for patients
- Monitor and evaluate patient progress and adjust care plans as needed
- Provide support and guidance to patient to help them adjust to changing circumstances
- Assist patients to access community resources, and provide education on available services
Case Manager Skills:
- Ability to assess patient needs

- Knowledge of relevant legislation, regulations and policies
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team

Case Manager Requirements:
- Master's degree in social work or psychology, or related field
- Knowledge of medical/mental health diagnosis, treatment and referral procedures
- Ability to establish and maintain effective working relationships
- Experience working with diverse populations
