What does the Manager of Community Transitions Clinical role entail?
General Functions: The Manager of Community Transitions/Clinical works with the interdisciplinary team to supervise and direct transitions, focused on the assessment and goal setting for individuals moving from nursing homes to community residential placements.
Specific Functions:
Oversee and provide clinical direction to the assessment and development of comprehensive service plans for each individual in the program.
Provide clinical oversight and sign-off for assessment and service plans as completed by staff in program.
Ensure that individuals are provided services and provide ongoing support related to individual service plans.
Assist team with assessments, transition plans and services as needed.
Oversee the development, coordination, and documentation of individual transitions with established documents, timeframes, and criteria.
Complete all required assessments, tracking data, reports, required trainings, visits to nursing homes, and provide transportation for individuals as needed during the transition coordination period as well as post transition.
Using a team approach, communicate the results of the screening and intake process with supervisor and other appropriate managers.
Participate in required weekly team meetings, case reviews, and monthly clinical supervision meetings; provide ongoing clinical supervision to assigned staff; and participate in continuing education to maintain applicable licenses/certifications.
Work in conjunction with other department supervisors to assure smooth transition into residential programs.
Facilitate agency trainings and participate in agency committees as appropriate and assigned.
Meet or exceed assigned weekly billing quotas as determined by the Director of Professional Services.
Maintain a driving record and automobile insurance in compliance with agency policy.
Participate in all agency and department trainings, meetings, online trainings, and any additionally recommended trainings or meetings as required.
Maintain compliance with all certification requirements, such as First Aid/CPR, Safety Care, etc.
Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
Other duties as assigned by the Coordinator of Community Transitions and the Director of Professional Services.

Qualifications: Consistent with requirements for Licensed Practitioner of the Healing Arts (licensed clinical psychologist (PhD), licensed clinical social worker (LCSW), or licensed clinical professional counselor (LCPC). Must possess master's degree in related field to be considered. CADC and co-occurring experience strongly preferred. Experience should be progressively responsible and should include at least five years in services for individuals with mental illness. Working knowledge of DHS/DMH
