Position Summary
Responsible for the provision of social services to facility patients and their families and will be directed in supporting and maximizing the adjustment, social functioning, and rehabilitation of the patient. These services will be provided in accordance with an established physician plan of care, and under the supervision of a registered nurse (and other medical professionals as dictated by state law).
Qualifications
· Candidate must possess a degree from an accredited 2- or 4-year college / university with evidence of the successful completion of a master’s program in social work from a graduate school accredited by the Council on Social Work Education.
· Non-master’s renal social worker’s may be grandfathered in under 405, subpart U prior to September 1, 1976 qualifying under specific criteria.
· Currently licensed in the State of Texas.
· Must have at least two year experience as a social worker (one of which must be clinical experience at a dialysis facility).
· Possess a valid State of Texas driver's license
· Candidate must have the ability to meet minimum facility hiring standards, including evidence of current cardiopulmonary resuscitation (CPR) certificate (and a willingness to perform such procedures if necessary).
Duties and Responsibilities
· Conducts psychosocial evaluations monthly and as needed.
· Participates in a team review of the patient’s progress monthly (Patient Care Conference).
· Participates in QAPI activities (Quality Assurance and Performance Improvement).
· Recommends changes in treatment (based on the patient’s current psychosocial needs).
· Provides casework and group services to patients and their families (in dealing with special challenges associated with ESRD).
· Identifies community social agencies and other resources that will assist patients and their families.
· Maintains and submits required documentation relative to services delivered to the patient (within the guidelines of facility policy).
· Participates in the collection of objective and subjective data to assess psychosocial status will occur within 2 weeks or 7 treatments from admission to the facility.
· Completes a comprehensive assessment (with an educational component) within 30 days or 13 treatments from patient’s admission to the facility.
· Completes an annual reassessment (or more often, if ordered by the Administrator or other appropriate staff).
· Provide psychosocial services at the facility during scheduled treatment times.
· Coordinates efforts to meet the psychosocial needs of the patients.
· Educates patient and family members concerning financial responsibility for home dialysis services.
· Monitor insurance needs of the patients in collaboration with finance department.
Job Type: Full-time
Pay: $70,000.00 - $75,000.00 per year
