What You Will Own…
Providing and directing cares to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and policies. Coordinating the total plan of care with other clinical professionals and helps achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
As the primary point of contact for our clients, you are personable and understand their sense of urgency and how to balance their demands with the existing daily deliveries.
Full Job Description:
We looking to add a weekend RN to our team. This would be Friday 5pm to Monday at 8am. Evaluate and treat home health patients under plan of care orders and supervision of a licensed physician to assist patients in reaching their goals in collaboration with the interdisciplinary home health team.
Qualifications:
- Current license/registration by the state of hire as a Registered Nurse
- Preferred one year experience in Nursing
- Prior Home Health Experience also preferred but not required
- Ability to complete Competency and Proficiency Skills Evaluation
- CPR Certification
- Excellent verbal and written communication
- Valid drivers license with a clean driving record and a clean, functioning vehicle
