Pay: $29.00 - $37.75 per hour (dependent upon type of license), credit for experience
$4.00/hour in shift differentials, plus an extra $1.50/hour on weekends!
About the Position
Londonderry Village is committed to providing person-centered, individualized care and services to the elders we serve. In the LPN/RN Unit Nurse position, you will coordinate and ensure quality nursing care is provided to elders, as well as instruct Nursing Assistants on medical care and issues.
Essential Duties and Responsibilities:
- Performs various duties in order to ensure quality nursing care is provided to elders. Ensures that care provided is focused on maintaining or attaining the highest practical level of functioning.
- Evaluates elder's condition and analyzed information obtained in order to develop appropriate interventions to prevent decline.
- Records signs of change in condition and notifies appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, and other changes. Notifies physician of changes in elder's condition.
- Notifies appropriate staff of need for restorative nursing evaluation based on change in condition.
- Administers medications and treatments to elders as per physician order. Documents administration in electronic health record (EHR). Informs physician of any adverse reactions to medication.
- Evaluates elders upon admission to develop baseline Care Plan and further assesses elder to contribute to comprehensive Care Plan.
- Attends and participates in Care Plan meetings as needed; updates Care Plan as needed.
- Responds to emergencies; evaluates the condition of the elder; decides on the type of medical attention required consistent with elder's Advanced Directive; calls appropriate ambulance service if needed; notifies physician when a change in condition occurs; notifies family or Agent under a Power of Attorney when elder is transferred or admitted to the hospital.
- Confers with elders and families about care; makes referrals to other departments when necessary.
Qualifications:
- Valid LPN or RN License
- Leadership experience
- Strong background in patient/resident care and supervision
- CPR certification (preferred)
- Proficiency in EMR and EHR systems
Shifts available: 3pm to 11pm (32 hours/week); 11pm to 7am (8 hours/week)
Job Type: Part-time
Pay: $29.00 - $37.75 per hour
Expected hours: 8 – 32 per week
