PACE Registered Nurse (RN)
Location: Lexington, KY
Schedule: Monday-Friday 8am-4:30pm
As a BCN Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. About the role:
The PACE RN performs all activities related to Registered Nurse services to achieve program goals for the participant at the PACE Center and the participant’s home as needed. The PACE RN is responsible for developing a plan of care based on nursing processes and implementation of Physician orders, while coordinating with other staff members as necessary. The PACE RN is also responsible for administrative and clinical tasks such as documentation of all visits, reviewing medical records, services as a primary point of contact for Infection control for PACE, and facilitating communication with key staff, participants, and/or their family members.
Requirements:
- Is a graduate of an accredited school of professional (registered) nursing.

Is currently licensed as RN through the State Board of Nursing.

Must have at least one year of experience working with the frail and elderly.

Food handling permit, to be completed upon hire.

Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.

Possesses and maintains current CPR certification.

Is self-directed with the ability to work with little supervision.

Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Must comply with requirements of other facilities (hospitals, LTC facilities, etc.) related to flu or other vaccines if job duties require providing services in those facilities.

Team Members will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
