Position: African American Health Conductor

Background
The Center for Human Development is a community-based organization that offers a spectrum of services for at-risk youth, individuals, families, and communities in the Bay Area. Since 1972 we have provided wellness programs and support aimed at empowering people and promoting positive growth.
The African American Health Conductor Program is an innovative partnership between Contra Costa Health Services and the Center for Human Development. The goal of the African American Health Conductor Program is to improve access to health care and reduce health disparities among African Americans living in Contra Costa County, including the re-entry population.
Job Description
The African American Health Conductors will be part of team serving African Americans in Contra Costa County. The Health Conductors create a bridge for African American families to the community and public systems that have traditionally been difficult to access due to their complexity, cultural capacity, and trust level. The Health Conductor will be assigned to the Pittsburg Health Center (East County area).
Duties & Responsibilities
- Conduct community outreach and referrals with a warm hand off;
- Provide patient navigation, i.e. assist with health access (Medi-Cal); Cal Fresh, social needs connections/resources.
- Develop an outreach campaign tailored for uninsured African Americans, located in the Pittsburg and Antioch area of Contra Costa County.
- Provide monthly data on the number of African Americans helped with program services.
- Make calls to patients to remind them of visits/appointments and to see how they are doing.
- Assist in designing promotional materials, as needed.
- Attend community events to promote programs and services.
- Attend staff meetings for the African American Health Conductors, the Center for Human Development, and important community meetings.
- Enter patient information and social needs resources into the electronic medical record database (EPIC);
· Duties may require traveling to different locations throughout Contra Costa
· Other duties as assigned.
Knowledge, Skills, and Abilities:
- Knowledge of African American community health promotion and prevention principles;
- Knowledge of the West County African American community and their main health and social issues;
- Knowledge of Medi-Cal application process
- Strong active listening, verbal, and written communication skills.
- Ability to organize and set priorities and manage multiple tasks;
- Ability to work independently and as part of a multi-cultural team;
- Ability to recruit and motivate program participants;
Education, Credentials and Experience:
- Work experience or even volunteer experience in community programs highly desirable.
- One to two years of experience working with African American communities, with knowledge of diverse areas of Contra Costa a plus; specifically East Contra Costa County.
- Computer proficiency sufficient in MS Office and Data programs highly desirable
Special Requirements:
- Occasional evenings and weekends as requested;
- Valid California Driver’s License (Required)
- Reliable transportation and proof of insurance (Required)
- Successful background check (Required)
- Clear T.B. test. (Required)

Salary
$23.75. hourly rate
Benefits
Include medical, dental, 19 days of vacation, 12 holidays, sick hours, and a retirement plan, which are all pro-rated based on FTE. This is a non-exempt position, 40 hours per week.
