We are looking for a Mental Health Case Manager for flexible full-time employment. This position includes providing services by telephone and video conference, working in the office and visiting clients in the community and school. This position will work with clients diagnosed with mental illness or both mental health and substance abuse disorders. The Mental Health Case Manager is responsible for the provision of ongoing community support services for a caseload of individuals. Both child and adult caseloads are available.
Experienced/Degreed Mental Health Case Managers provide Therapeutic Behavioral Services (TBS) which are goal-directed supports and solution-focused interventions to persons diagnosed with a mentally illness; and develop individualized treatment plans with their clients. Mental Health Case Managers work with clients, family, support networks and other healthcare professionals supporting clients with making progress toward the goals of their individualized Treatment Plans. They provide the client with on-going monitoring, support, and training in self-care, social integration, skill-building and health maintenance. They help clients to identify and mobilize supports that will be responsive to the clients’ individualized needs. Mental Health Case Maners help clients to identify and minimize problems which interfere with the client’s financial management, healthy personal development, and school or job performance.
Qualifications:
- Must have a bachelors or a masters degree in social work, psychology, nursing, or in related human services; or a high school diploma with a minimum of three years of relevant experience. Relevant experience could include, but is not limited to, one or more of the following:
- Service/treatment planning, care coordination, and/or referral and linkage for healthcare, behavioral healthcare, or non-healthcare services
- Community Support of persons with mental health, substance use concerns, and /or developmental disabilities
- Intervention using evidence-based techniques
- Identification of strategies or treatment options
- Restoration of social skills and daily functioning
- Crisis prevention and amelioration
- Establishment of trust and rapport, active listening, empathy and goal-setting
- Social, spiritual, educational, and/or other supportive, relational activities
- Must have a valid Ohio Driver’s License with a safe driving record and valid insurance.
- Ability to take and pass a physical exam, tuberculosis test and drug screening.
- Employment is contingent upon clear results of a thorough background check.
