Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program.
As a Senior Case Manager, you would be providing professional, personalized, and specialized referrals, linkages and coordination of services to individuals with behavioral health issues. This role serves as a "safety net" for the individuals served, therefore, the incumbent must be flexible, solution driven, innovative and compassionate. The incumbent must have the skill and professional capacity to function independently in the community. Participation at community meetings, events and activities and aggressive outreach to other service providers and individuals is required.

Qualifications:

Master's Degree in human services field with at least 1 year experience in mental health services preferred OR

Bachelor’s Degree in Social Work, Psychology, Counseling, or Criminal Justice with three years Mental Health experience, one of which must be in Service Coordination.

Valid Driver’s License, this is a mobile case management position and driving is required.
