Job Description
o Engages in assessment, treatment, education and consultative activities for patients, primarily on the inpatient unit, who may have a substance abuse problem
o Develops appropriate evaluation rep01ts and/or psycho-social/addiction histories for new patients.
o Documents findings, interventions and plans in patients' medical records on the same date patients were interviewed;
o Coordinates appropriate referral(s) to treatment programs/facilities, including the provision of relevant information about professional services, community resources, and appropriate follow-up communication;
o Participates in CATCH/ED Leads teams' unit interdisciplinary team rounds and provides updated information about the patients to the team;
o Provides information related to CATCH/ED Leads inpatient consultation services to patients and next of kin as needed or as required by the interdisciplinary team
o Provides consultations to other Hospital Personnel about CATCH/ED Leads services and available treatment facilities;
o Provides therapeutic services and conducts educational groups dealing with the disease of addiction, transition of recovering persons, etc.
o Negotiates and provides direction to patients regarding contingency contracting (appointment scheduling, screening mechanisms, etc.).
o Provides home visits, follow-up and aftercare, as necessary.
o Reports directly to the Behavioral Health Department
o Can be assigned to other designated areas in the hospital as needed when deemed appropriate by supervisor/designee;
o Responds and participates in external and internal disasters when indicated/assigned by supervisor.
o Esco1ts patients for placement interviews/discharge when appropriate and/or indicated by supervisor or CATCH/ED Leads provider lead
o Quality Assurance Performance Improvement (QAPI) coordination for all Behavioral Health related issues.
o Participates in the formulation of standards, policies, and procedures.
o Provides orientation to new staff.
o Provides information about and acts as linkage to available community resources.
Minimum Qualifications
Assignment Level I
1. A valid License or Limited Permit issued by the New York State Education Department to practice as a Mental Health Counselor.
Assignment Level II
1. A valid License issued by the New York State Education Department to practice as a Mental Health Counselor; and
2. Two years of full-time, paid, professional experience providing direct client care in a
behavior health setting.
Assignment Level III
In addition to meeting the requirements for Assignment Levels II, Assignment Level III
requires:
1. Three additional years of post-licensure experience..
