The Quality Manager (QM) is responsible for overall management of the Quality Assessment Performance Improvement (QAPI) program for Family Hospice Care (FHC). The QM collaborates with members of the Management team to develop, implement and maintain effective, on-going date-driven processes and programs which meet regulatory compliance and maintain FHC standards of practice.
- Responsible for development, implementation and maintenance of the QAPI program.
- Implement, monitor, and evaluate performance improvement activities using continuous quality improvement principles and methodologies.
- Performs data analysis and identifies opportunities for improvement.
- Manages and coordinates all QAPI data collection activities and databases.
- Compiles, trends and reports quality data, following the QAPI Program Plan. Creates and coordinates presentation to QAPI Committee and the Leadership Team. Collaborates with Administrator in creation and presentation of annual evaluation to the governing board.
- Actively participates with leadership in clinical record review to monitor compliance with Conditions of Participation, Clinical Outcomes, and Standards of Practice.
- Responsible for CMS mandated reporting of Hospice Quality Measures & the analysis of Hospice CAHPS survey results.
- Coordinates reporting and tracking of unusual occurrences and adverse events (including Ca Department of Health reporting as mandated). Performs causal analysis and facilitates performance improvement activities.
- Responsible for preparation and participation with compliance surveys and medical reviews e.g. state surveys, ADR’s.
- Maintains current knowledge related to Medicare Conditions of Participation, regulatory compliance and standards of practice; coordinates updates to leadership team.
- Responsible for the development and annual review of all Hospice Policies & Procedures in collaboration with Leadership Team.
- Educates Hospice staff on an on-going basis with updates to Conditions Of Participation, regulatory, compliance and standards of practice.
- Collaborates with Administrator in the Implementation and Maintenance of; FHC’s Compliance Program, HIPAA Privacy Rule (accountability for all matters related to compliance with Privacy Rule), HIPAA Security Rule (accountability for all matters related to safeguarding the confidentiality, integrity and availability of electronically protected health information) and designates responsibilities and duties, as appropriate.

Qualifications
- RN with current licensure to practice professional nursing in the state of California. Bachelor's degree in nursing preferred.
- Minimum 5 years experience in a health care environment.
- Moderate to advanced knowledge of QAPI principles and methodologies.
- Basic computer skills (Word, Excel) and interpersonal skills.
- Able to manage self and others in a team-oriented, collaborative environment while meeting deadlines and achieving targets.
- Valid California driver's license and proof of automobile insurance.
