Title: PACE Nurse Practitioner
Schedule: Monday-Friday, 8am-5pm
On-call responsibilities: Our NPs are "on-call" one day per week during their regular work week to assist our RNs with patient facing needs as they arise. Our NPs also rotate the weekend on-call schedule every 6 weeks. It's our goal to provide our team with a work/life balance.
Pay: $113,451 to $136,975 per year
Sign-On Bonus: $6,000
JOB SUMMARY
The Nurse Practitioner works in collaboration with physicians and interdisciplinary team as an advanced practice nurse to deliver primary care and other health services to participants enrolled in PACE.
The PACE Nurse Practitioner may also participate in clinical call rotation for TRU Community Care and provide care to hospice patients.
ESSENTIAL DUTIES
- Provides diagnosis, treatment and management services for PACE participants and hospice patients as directed
- Participates actively in interdisciplinary team conferences, helping to develop and revise participant/patient care plans and acting as a medical resource in determining approaches to individual participant/patient care
- Responsive to provide urgent visits in the clinic or in the participant home when required during work day and when on call during the day or on the weekend
- Works in a safety conscious manner which insures that safe work practices are used in order not to pose a risk to self or others
EDUCATION AND QUALIFICATIONS
- One or more years of working experience as an NP
- Board Certified Advanced Practice Nurse in the state of Colorado
- Controlled substance license with current DEA license number
- Eligible for or currently possess a Medicare/Medicaid UPIN number
- Eligible to bill insurance for palliative care and symptom management visits
- Experience working with frail/elderly (preferred)
- Must have $1M/$3M professional liability insurance and TRU will reimburse the premium costs
- Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100k Property Damage coverage), and have reliable transportation
- Must comply with agency health screening requirements, including medical clearance for all communicable diseases prior to evidence of direct participant contact, and proof that all immunizations are up to date prior to direct participant contact
