**Job Summary:**
- May participate on the admission team, with the Administrator, Director of Nursing and Admissions Coordinator to assess the appropriate of referrals for admission to the facility. - Participates in the formation of the total care plan for each resident based on medical, nursing and psycho-social needs of the resident. Resident and family will be involved to the extent feasible in formulating the care plan
- Develops the social services evaluation for the clinical record, which may include the resident’s social history, social and emotional needs of the resident, working goals and treatment plan
- Liaisons with family members and responsible parties
- Serves as an advocate for Residents’ Bill of Rights
- Coordinates with outside agencies, i.e., mental health organizations, to meet psychosocial and emotional needs of residents - Implements working knowledge of Medicare and Medicaid bylaws.
- Must be licensed by the Commonwealth of Massachusetts
- Must have at least a BA in Social Work - At least one (1) year in management/supervisory role preferred
- At least two (2) years’ work experience as a Social Worker required with at least one (1) year in Long Term Care facility preferred
- Possess working knowledge of state and federal programs and regulations affecting nursing homes
- Possess general knowledge of community health and social resources - Knowledge of growth and development across the life span
Job Type: Part-time
Pay: Up to $41.00 per hour
