SUMMARY: The Direct Support Professional provides personal support services and other scope of services as directed in the Individual Service plan/Healthcare plan. Complete daily activity notes, to document the services provided to the Individual, for the assigned schedule. Perform duties in a professional manner. Be sensitive and respectful to the individual culture and diversity.
SKILLS COMPETENCIES: Provide services and support; ADL’s, Nutrition, Safety, Infection control, Observation, Communication, Customer Service, Documentation, Professionalism.
DUTIES AND RESPONSIBILITIES: follow each Individual’s Service Plan/Healthcare plan that requires the services below. Document personal support service delivery with the number of days and time an Individual receives services according to the DSP’s schedule.
Personal Care Tasks:
· Bath (e.g., bed, tub, shower)
· Routine Skin Care
· Hair Care (Comb, brush, shampoo)
· Oral hygiene (brush, floss, dentures)
· Assist with dressing/grooming
· Assist with continence/incontinence
· Nail care-filing prn, NO clipping
· Skin care/Shave (DO NOT SHAVE A DIABETIC INDIVIDUAL)
· Assist with activities of daily living (ADL)
Homemaking Tasks: (Only the Individual’s area)
· Follow care plan for tasks to be performed.
· Make bed, change linens as needed/when asked
· Vacuum, sweep, dust, mop, laundry, empty trash
· Clean bathroom, wash dishes
Nutrition/ Meal preparation
· Prepare and serve a light meal and/or snacks
· Encourage proper nutrition and follow the Individual diet plan
· Assist with Eating/ Clean Up
· Observe and report meal accumulation, follow proper food storage
· Observe and encourage fluids
Mobility:
· Assist with transfers – (e.g., in and out of bed, vehicles and/or wheelchair)
· Use Hoyer lift for transfer (if applicable)
· Assist with ambulation using special equipment
· Encourage physical activity as condition permits
· Position the Individual/ROM
· Maintain comfort and safety to the Individual
Health Care Activities:
· Catheter care with soap and water
· Medication reminder, monitor Individual with self-administration of medication
· Take Vital signs
Observation, Report and Documentation:
· Observe, report and document Vital signs
· Document care notes in Generations/Individual’s binder before departing
· Observing/Reporting any change in the Individual’s condition to Nurse Supervisor
· Report any unknown abuse or suspected abuse to Nurse or Supervisor
· Report any unsafe condition in home to Nurse or Supervisor
· Report any incidents of the Individual
· Providing watchful supervision and oversight
Safety/Emergency Instructions
· Make emergency calls for the Individual, family or designated person, when needed
· Call 911
· Perform CPR/AED/First Aid (if applicable)

· Basic Understanding of Tuberculosis and Identifying and Reporting Potential TB Exposure.
· Review/Understand Family Bridge Policies and Procedures
Companion/Sitter -Duties and Responsibilities:
· Follow Nutrition/Meal preparation
· Follow homemaking tasks
· Follow Observation, Report and Documentation tasks
· Follow Safety/Emergency instructions
· Assist with ambulation/ROM
· Engage Individual with activities and communication
OTHER DUTIES AS ASSIGNED
· File, telephone communication, fax, shred, and correspond with emails daily.
· Conduct and record live fingerprint scan for background.
- Complete referral forms for services of potential individuals and fax to Agency/ Case Management Services.
· Other duties as required to maintain the overall operation of Family Bridge Healthcare.
QUALIFACTIONS
· Certified on the Georgia State Registry.
· Health related training courses or equivalent combination of education and experience.
· High School Diploma or GED.
· Clear DBHDD and/or GCHEXS criminal background check.
· Current CPR/First aid certification (must be renewed every two years).
· Current annual health assessment examination.
· Current PPD screening or chest X-Ray/TB screen (updated annually).
· Appropriate documents for I-9 form.
PHYSICAL REQUIREMENTS
Ability to lift up to 50 pounds
