The Transitional Services Supervisor is responsible for all re-entry aspects of the members served. The Transitional Services Supervisor is charged with the management and direct oversight of the Transitional Services Re-Entry Specialists and Peer Coaches. This position is responsible for performing a wide variety of member support activities as directed by the department Director, Chief Clinical Officer, and/or executive team members. The schedule for this position is 8:30am - 7:30pm Friday - Monday - this position qualifies for swing shift differential.
Duties and Responsibilities:
- Leads employees within their direct reporting structure who are responsible for daily re-entry/peer coach activities of the facility and organization by ensuring that quality member-centered care is delivered in an efficient and effective work environment
- Effectively balances member support activities while overseeing the delivery of member care, coordinating admissions, discharges, and transfers with the appropriate staff, teams, and departments
- Collaborates with the department Director, Human Resources, and members of the executive team, to lead, create, and maintain an environment that advocates for staff and promotes individual/team engagement through clinical/administrative expertise and professional leadership
- Achieves department/organizational outcomes, effectively aligning and managing to the company’s metrics
- Takes an active interest and accountability for key operational functions including but not limited to member safety issues, timely resolution of complaints, quality improvement initiatives, survey preparation, and client satisfaction follow-up
- Takes responsibility and ensures accountability across the department to adhere to department/organizational policies and procedures, compliance, and regulatory agency requirements
- Establishes professional relationships with compassion and respect for human dignity and the uniqueness of individuals, while demonstrating a primary commitment to the member in advocating for member privacy, health, safety, and rights
- Supports a work environment that focuses on staff, customers, and client/family through safe and effective communication
- Serves as a daily operational resource to staff and in the management of supplies, equipment, tools, and resources to fulfill job responsibilities
- Provides direct oversight to Transitional Services Re-Entry Specialists and Peer Coaches by coaching staff for ongoing professional development; effectively navigating boundary and/or performance challenges; providing ongoing support and assistance; maximizing staff retention and employee satisfaction; partnering with Human Resources for leave of absence needs and/or accommodations; evaluating staff performance; and managing staff time off requests and timekeeping
- Manages the recruitment, selection, hiring, onboarding, and training of staff
- Demonstrates core competencies including but not limited to knowledge of recovery, ethical and boundary-related dilemmas, reasonable accommodations, advocacy skills, and self-disclosure
- Consistently provides and documents strength-based clinical oversight and supervision of designated staff, through a combination of individual and group supervision
- Monitors and signs off on staff documentation and billing, ensuring accurate, thorough, and best practices
- Collaborates professionally with courtesy and tact when interacting with the leadership team, employees, and members
