The Black Brain Campaign is a non-profit 501c3 organization created to eradicate the stigma against mental health in the Black community by providing a holistic approach to therapy treatment. Our services include free individual and family counseling, training courses for therapists, supervision, and community resources.
JOB SUMMARY:
The Mental Health Case Manager plays a crucial role in our clinic, providing comprehensive case management services to individuals seeking mental health support. This position involves assessing client needs, coordinating services, and collaborating with a multidisciplinary team to ensure effective and personalized care.
KEY RESPONSIBILITIES:
1. Conduct thorough assessments to identify client needs, strengths, and goals.
2. Develop and implement individualized care plans in collaboration with clients and the treatment team.
3. Coordinate and advocate for appropriate mental health services, including therapy, medication management, and community resources.
4. Monitor and evaluate client progress, adjusting care plans as needed.
5. Facilitate communication among clients, families, and various service providers to enhance the continuity of care.
6. Assist clients in navigating healthcare and social service systems.
7. Provide crisis intervention and support during emergencies.
8. Maintain accurate and up-to-date case records, documenting assessments, progress notes, and relevant information in compliance with regulatory standards.
9. Collaborate with other mental health professionals, such as therapists, psychiatrists, and social workers.
10. Stay informed about relevant community resources and mental health trends.
QUALIFICATIONS:
· Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a related field.
· Relevant experience in mental health case management.
· Knowledge of mental health diagnoses, treatment modalities, and community resources.
· Strong communication, interpersonal, and organizational skills.
· Ability to work collaboratively in a team-oriented environment.
· Empathy and a genuine commitment to improving the mental health and well-being of clients.
· Certified Case Manager (CCM) preferred.
· Valid FBI and child abuse history clearances are required.
