Summary
The Social Services Case Manager is a full-time position primarily responsible for providing case assistance, benefits counseling, advocacy, and therapeutic groups to participants in the Older Adult Center and Selis Manor Community program. The Social Service Case Manager will be responsible for the coordination of case assistance provided by others.
Essential Duties and Responsibilities include the following:
- Offer a minimum of 2 therapeutic groups per week.
- Submit 25 monthly case assistance notes to the Director of Older Adult Center for client tracking/reporting procedures and 20 information units per month.
- Complete intakes in the OAC and Selis Manor programming.
- Assist with the development and/or facilitation of health promotion and socialization services including the following: physical health/exercise, health management, mental health programs, arts & culture, technology, and education programs.
- Provides at least 1 weekly workshop in the OAC that will increase the health management unit.
- Cover groups in OAC when necessary. (i.e., when facilitators are out or on vacation.
- Responsible for promoting and publicizing program activities for Older Adult Center, via tours of the facility and addressing daily walk-ins.
- Participate in outreach/network opportunities to publicize VISIONS at Selis Manor and the Older Adult Center to recruit youth volunteers and senior participants.
- Responsible for placing a minimum of 20 outreach calls per week to promote OAC programming and/or meal service.
- Recruit volunteers to ensure a minimum of 15 case assistance units are submitted monthly.
- Adhere to safety, health standards and guidelines of the Older Adult Center.
Coordination of Case Assistance & Tech services through Intergenerational Program:
⮚ Ensure 8-10 (units) case assistance is provided by the IGP participants through community excursions.
⮚ Manage implementation and scheduling of a minimum of eight intergenerational sessions (meals, classes, workshops, community excursions) to support the meeting of the Ed/Rec Cost Center units.
⮚ Recruit, screen (secure three references) hires and train 20 + youth volunteers (15 and older) for the intergenerational program. Write a written evaluation for each youth at the end of each semester.
⮚ Recruit 50 older adults for technology training skills.
⮚ Assist with pretests and posttests service assessments of the intergenerational technology training.
⮚ Keep records of and monitor intergenerational services provided.
⮚ Supervise youth volunteers recruited to help with OAC operations.
⮚ Coordinate any grant sponsored services as needed related to older adults and youth.
⮚ Track youth volunteers, hours, and school partnerships.
⮚ Coordinate bimonthly meetings with student coordinators to ensure progress and/or issues are addressed.
⮚ Participate in intergenerational meetings.
Community Outreach and Public Relations:
- Assist with community presentations about Older Adult enter programs.
- Assist with the recruitment of older adult participants.
General Duties:
- Plan and develop Selis Manor special events that promote all the services within the community center and support the program goals.
- Ensure that all policies and procedures are adhered to throughout VISIONS at Selis Manor
- Follow and adhere to incident reporting and safety procedures, complete incident reports when necessary.
- Responsible for at least one (1) late night per week to close the facility. This will fluctuate based on program needs.
- Perform other duties as required.
Education and/or Experience
- Master’s degree in social work or related field, LMSW.
- 3+ years of relevant experience in case management, social work, or a related field
- Ability to accept guidance and supervision.
- Ability to work independently in a collaborative environment and take initiative.
- Maturity, integrity, adaptability, enthusiasm, and reliability. Demonstrate a positive attitude.
- Strong organizational, time management and documentation skills
- Excellent communication, interpersonal and counseling skills
- Work efficiently and with accuracy in a fast-paced, deadline-driven environment.
- Strong understanding of human services, social systems, and community resources
- Must be flexible with work schedules to meet the needs of programming, training/workshops, community presentations etc.
- Proficiency in electronic database management and Microsoft Office
- Strong communication skills.
- Ability to speak a second language is a plus.
- Sifi certification is preferred.
SKILLS AND REASONING ABILITY: Attention to detail, ability to work independently, ability to interact with staff and vendors, must be a self-starter, well organized, a team player and understanding of the philosophy of a nonprofit service organization and the impact that it has on the culture and operation of the agency. Excellent written and verbal communication skills.
Salary: $68,000-$80,000 Commensurate with education and experience.
Location: The center is located at VISIONS at Selis Manor, 135 West 23rd Street, New York, NY 10011. This role requires you to be onsite no less than 4 days a week unless otherwise advised by the supervisor. This role also requires covering evening events as needed.
