Position Summary
Under the direct supervision of the Director of Community Programs, the Program Manager/Case Manager for Individual Pathways (IP) will implement the overall service design and coordinate services and supports for participants at multiple locations in the community. This role involves staff supervision, development, program planning, implementation, and case management to ensure agency, program, and participant goals are met. Provide case management services which include case record keeping, advocacy, and service coordination for consumers (adults with varying degrees of learning, behavioral, emotional, and developmental disabilities) in the program. Additionally, performing instructor duties in the community is necessary to maintain staffing ratios.
Essential Duties and Responsibilities

Oversee Individual Pathways Program and provide leadership in program development, make recommendations for improving operations and services. Maintain program standards as required by regulatory agencies. Provide direct service in program operations as needed to assure positive outcomes and adequate staffing ratios.

Oversee all personnel actions including hiring, evaluations, discipline, and terminations; provide training, development, and orientation of staff in compliance with established ECF policies and procedures. Coordinate staff schedules to maintain coverage and ensure support services are being provided according to service design. Ensure that Time and Attendance procedures are followed daily. Participate in team meetings, safety committee meetings and in-service training. Assist in providing in-service training to other direct service staff. Maintain records as required by regulatory agencies for the operation of the program.

Maintain maximum participant enrollment and daily attendance ensuring the accurate completion of monthly e-billing. Develop and manage department budget and monitor and analyze monthly reports of income and expense to assure targets are maintained. Provide fiscal and grant management.

Develop curriculum, instructional materials, community awareness, while implementing, coordinating, and evaluating program services, goals, and objectives. Complete monthly reports and submit information for newsletters. Keep program flyers updated. Develop partnerships and maintain external relationships with funders, families, other agencies, and businesses. Conduct tours and perform presentations as needed.
- Review all client referrals and make enrollment decisions. Provide tours and attend vendor fairs for client recruitment. Develop individual service plans for participants to achieve greater independence. Monitor participant progress, provide crisis intervention, and assist in behavior management programs. Maintain external relationships with funders, families, agencies, and businesses through tours and presentations.

Maintain complete case records, notes, and paperwork according to regulatory standards. Ensure documentation compliance with CARF, Licensing, Regional Center, DDS, and ECF standards. This includes case notes, Consumer Individual Risk Assessment forms, evidence of negative TB tests, physicals, etc.

Establish and maintain good working relationships with consumers, parents and/or care providers, and referral/service provider agencies. Coordinate and conduct annual ISP meetings with participants and circle of support to include parents, care providers, significant others, regional center representatives and other emergency meetings as necessary.
Other Duties

Adhere to ECF’s Injury & Illness Prevention and Safety Program. Ensure safe use and operation of tools and/or equipment. Report work related injuries/illness that involve consumers enrolled in the programs with a paid component.
- Assist facility staff during emergency medical situations include implementing First Aid, CPR, NCI, and transporting of consumers to home or medical facility if necessary.

Comply with ECF policies and procedures, including mandated reporting requirements. Implement ECF guidelines as they relate to medical emergencies, suspected abuse or neglect and unusual incidents.
- Observe regular, on-site work hours.

Perform other duties as assigned.
Qualifications
Education/Licenses/Certifications: Bachelor’s degree in human services Field required. 8 years of related field experience in lieu of a degree.
Experience: Five years of professional experience working with adults with special needs, preferably with adults with developmental disabilities, of which 2 years must have been in a supervisory capacity.

Requirements

Valid California driver’s license, reliable transportation, evidence of car insurance, and insurable driving record. Motorcycles are not permitted while driving for ECF, may only be used when commuting to and from your home to main worksite.

Pass a DOJ background clearance.

Provide evidence of a negative TB test initially and every three years thereafter.

Obtain NCI, First Aid and CPR certification within the first 3 months of employment and maintain as required.
Work Environment
This is a program for adults who have varying degrees of learning, behavioral, emotional, and developmental disabilities. Employee may encounter consumers who are in crisis, exhibiting emotional/psychological distress or behaving in an aggressive manner. Work will be performed in a shared office and at multiple community and volunteer sites. Office is located in a building with climate-controlled temperature. Will use own car, ECF vehicle, or public transportation to perform duties. May encounter inclement weather.
