Fair Oaks Health Care Center has a great opportunity for the right person! We are currently seeking a MDS Coordinator who must be RN to oversee the facility staff with skills to facilitate change and build a team and a strong passion for quality patient care.

Qualifications:
- Current state licensure as a Registered Nurse licensed to work in the State of Illinois, which must be in good standing
- Three years prior experience in long term care, MDS/RAI, with a minimum of one-year supervisory experience
- Strong desire to work with the elderly
- CPR certified
- Knowledge of current state and federal regulations governing long term care facilities
- Ability to utilize Microsoft Word, Excel, Outlook and MatrixCare
- Interest in and good working knowledge of MDS, Medicare, Medicaid and care planning process
- Must have excellent interpersonal skills that promote effective communication with personnel, residents, family members, visitors, government agencies, and the general public
- Must be English literate and possess excellent written and verbal communication skills
- Must be a positive and upbeat individual that is able to handle case load of a 51 bed facility
- Must be able to maintain confidentiality (HIPAA)

Responsibilities:
- Supervise all levels of nursing personnel and direct resident care
- Assists in orientation and training of all nursing personnel
- To assure that residents rights are observed and respected through delivery of quality nursing needs
- Assesses the resident upon admission, discharge and on an on-going basis for nursing needs
- Interacts with resident, family members, guardians or other responsible parties to establish goals to achieve the individual resident’s optimal level of functioning
- Utilizes established health care goals, physician’s orders and nursing assessments to initiate an individual plan of care
- Follows established procedures to facilitate timely implementation of physician orders
- Administers meds and treatment to resident. Demonstrates knowledge of medications and treatments, the side effects, desired outcomes of meds and treatments and resident reaction to them
- Performs lab tests, monitors lab results and communicates results to physician as necessary
- Applies scientific knowledge in performing nursing procedures and techniques based on established principles and facility policies
- Reviews the care plan on a regular basis to ensure that resident goals are relevant and realistic for the individual resident, recommends changes and implements changes as necessary
- Delegates specific tasks to other care providers to ensure effective implementation of plan of care
- Completes weekly care summaries for individual residents through efficient use of care plan and care CNA charting
- Reviews CNA charting for timely completion and accuracy
- Monitors resident’s condition through effective assessment of symptoms, reactions and outcomes and records appropriately on designated forms. Is responsible to contact attending physicians and designated responsible parties as necessary to report changes of condition.
- Assist with resident’s education and rehabilitation to promote optimum mental and physical health
- Promotes therapeutic relationships with residents, families, nursing, ancillary departments and community agencies
- Provides an environment conducive to resident and employee personal growth and development
- Demonstrates leadership abilities to ensure effective implementation of the nursing process and quality, safe resident care
- Ensures compliance with Resident Rights as directed by policy and legal standards
- Demonstrates knowledge of facility disaster procedures and functions according to established facility policies
- Ensures residents dignity by daily quality assurance tours of the facility
- Reports needs of nursing department to the Director of Nursing
- Top be subject to call and carry cellphone on them on an as need basis
- To be responsible for the complete MDS/RAI process
- Knowledge of PPS and implementing and developing of MDS resident assessment and organizes interdisciplinary care meetings to identify individual needs with appropriate interventions.
- Knowledge of OBRA regulations and their applications to a Nursing Home Facility.
- Responsible for patient care assessments, planning integrating and implementing patient care after needs have been identified.
- Excellent ability to manage entire MDS process
Working Conditions:
- May be involved in tasks that involve exposure to blood, body fluids or mucous membranes on a daily routine basis
- May be required to work more than 8 hours in a day
