HIRING FOR 2 CASE-MANAGEMENT POSITIONS!
ESSENTIAL DUTIES AND RESPONSIBILITIES
- To coordinate psychiatric, psychological, medical, and dental services, and provide transportation when needed.
- To provide the full range of case management for youth.
- To provide vocational guidance, assistance, and referral.
- To make appointments for external clinical, evaluation, and specialty services.
- To attend all bridge, treatment and crisis meetings for clients and facilitate in the absence of the Program Clinician (and able to report out).
- To assist clinical team to arrange home visits and family contacts for clients.
- To complete clinical case management documentation on eHana and iFamilyNet and obtain MA-level sign-off.
- Assist in ensuring signatures of DCF worker, family members, and youth are obtained on treatment plans and progress reviews.
- To provide advocacy when needed for assigned youth and provide updated advocacy written updates to the youth’s treatment team/DCF worker.
- To keep files up to date, make sure progress reports are written and forwarded to appropriate others.
- Serve on the after-hours residential on-call rotation.
- Advocate for the development of additional programming resources for families served
- Serve as additional staffing coverage and support when needed.
- Obtain MAP certification and assist in Med Administration when needed.
QUALIFICATIONS:
- BA/BS in related field required.
- Knowledge of community resources and service planning.
- Excellent verbal and written communication skills.
- Exceptional time management skills.
- Valid driver’s license and vehicle.
EXPERIENCE: Case Management and/or Child-Welfare Service Coordination experience preferred.
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
