Type: Fulltime, Nonexempt
Anticipated Schedule: Five 8-hour day shifts per week. Weekend rotation of at least one weekend per month. One back up on call shift per month

Position Overview:
The Intake & Triage Nurse is a critical member of the hospice and palliative care team, responsible for managing the intake process for new patients, providing comprehensive and compassionate nursing care, and ensuring seamless communication and coordination among patients, families, health providers, and community resources. The Intake and Triage RN is pivotal in facilitating admissions, conducting assessments, and developing care plans in a timely manner, while also offering telephonic triage coverage to evaluate patient needs and determine appropriate levels of care. The Intake and Triage RN fosters relationships with key referral sources and participates in community outreach and education to raise awareness of hospice and palliative care services, enhancing the overall quality and accessibility of Tri-Cities Chaplaincy’s programs. The role requires a high level of clinical expertise, adaptability, and empathy to support patients and families during their end-of-life journey, ensuring that they receive the highest standard of care in various settings.
Requirements:
Minimum Qualifications:
1. Valid and unrestricted Registered Nurse (RN) License in the State of Washington.
2. Minimum of two years of experience in hospice, palliative care, or a related field; preferred.
3. Demonstrated ability to adapt to different work environments and provide effective care in diverse settings.
4. Excellent assessment, critical thinking, and problem-solving skills.
5. Strong communication and interpersonal abilities to establish rapport with patients, families, and healthcare professionals.
6. Ability to work independently, manage time effectively, and prioritize competing demands.
7. Compassionate and empathetic approach to caring for patients and families during their end-of-life journey.
8. Proficient computer skills and experience with electronic medical records systems.
9. Available e to work occasional evenings and weekends.
10. This position is designated as safety-sensitive and will require passing pre-employment drug testing, which includes cannabis testing in WA State.
11. As required per position/credentialing; successful completion of Blood Born Pathogens Training.
Preferred Qualifications:
Experience in healthcare, hospice, home health, hospital, and/or education.

Compensation
Pay Range: $35.00 – $73.81/hour
Starting Pay Range: $35.50 – $64.79/hour
Premiums:
- $2.50/hour Week Night
- $2.25/hour Week Day
- $4.75/hour Weekend Night
- $4.00/hour On Call Standby

At Tri-Cities Chaplaincy, we carefully establish salary ranges for our positions by taking various factors into account, including education, certifications, knowledge, skills, experience, and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.

- Mileage Reimbursement: $0.67 per mile, for eligible positions.
- Vacation: Accrual of hours each pay period based on hours worked.

AGENCY REQUIREMENTS
1. Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
2. Participate in community Education and Outreach to raise awareness of agency-provided programs including but not limited to hospice care, advanced care planning, and bereavement.
3. Responsible for knowledge and compliance with agency policies, procedures, and practices.
4. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patient-related information on a “need to know” basis only.
5. Reliable personal transportation, and current and valid driver’s license and vehicle insurance.
6. Satisfaction with state and agency requirements for Tuberculin screening.
7. Hepatitis B immunization series, proof of series, or signed declination.
8. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.
9. Must pass a criminal background screening and pre-employment drug screening.
10. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).
11. Complete agency and departmental orientation and training program(s).
12. Complete all continued learning, education, policy reviews, and required training by their respective deadlines.
13. Maintain licensure(s) and certification(s) required for the position.
14. Available to work as assigned/scheduled.
15. Perform other duties as assigned.
