JOB SUMMARY
Under the general supervision of the Vice President of Community Support, the Community Support Center Social Worker will take responsibility to ensure that our residents are connected to the supportive services that will enhance their quality of life and allow them to continue to live independently. The Social Worker will possess a genuine interest in and concern for geriatric, chronically ill and/or disabled persons. Must be able to fluently communicate in or to take or give direction or guidance in the English language.
PRIMARY ACCOUNTABILITY
The Community Support Social Worker must have hands on familiarity with the established practices and procedures as set forth by Glencroft and state and local requirements in facilitating community support services. He/she must have the ability to maintain good rapport with residents, their family and staff of the entire Glencroft Campus. The Social Worker will possess a good working knowledge of the community support resources both on campus and in the community. He/she must possess strong written and oral communication skills and be able to understand written and oral instructions.
ESSENTIAL FUNCTIONS

Regular attendance is considered an essential function of this position and is necessary for the efficient operation of the business.

Responsible for creating, communicating and implementing social service policies, procedures, and programs for independent living residents.

Meet with new residents upon move-in for introduction and follow-up in 30, 60 and 90 day time frame to ensure adjustment to the Glencroft community.

Provide individual resident counseling and resource support on an as-needed basis.

Responsible for facilitating resident and family requests for social service intervention and provide appropriate resources.

Track and visit at-risk residents, when necessary contact families and start the transitions process to a higher level of care.

Visit residents in hospital and rehab when deemed appropriate.

Establish and maintain a good rapport and open communication with residents and staff.

Responsible for monthly reports on social service activity.

Performs other duties as assigned.
MINIMUM QUALIFICATIONS
To function effectively in this position, the incumbent should possess a Bachelor’s Degree in social work, gerontology or a related field, with three years preferred experience in a health facility or geriatrics, or an equivalent combination of experience training or education is required. A thorough knowledge of community support resources in the community is preferred. The incumbent must possess excellent human relations and communication skills to provide effective working relationships, especially with department heads and supervisory staff. Must be computer literate. Must have the ability to simultaneously address multiple projects, demands and issues and make correct decisions in a busy operational environment. Must be self-motivated and be able to work effectively with little supervision. Must be able to interact with all residents using tact, compassion, patience, and courtesy.
